Health Tourism in the EU: Facts and Figures

What is health tourism, why is it growing so fast, the growth of health tourism, why do people travel for healthcare.

Health tourism has been around for thousands of years since the third millennium B.C. In fact, people in ancient Mesopotamia traveled to the temple of a healing god or goddess in Tell Brak, Syria to heal eye disorders. Greeks and Romans also traveled by foot or boat to spas all over the Mediterranean. And now people are traveling from all corners of the world to the EU to receive medical treatments, creating a market called health tourism.

Let us look at the state of health tourism in the EU, where it is headed, who the key players are, and how it could impact your tour operation.

Health tourism is a form of tourism that consists of patients traveling to other countries to get medical treatment or assistance. It comprises all the services associated with tourism like transport, accommodation, and hospitality. Health tourism consists of medical tourism, wellness tourism (to enhance health), and spa tourism (spas that combine medical and health components). The three components are different but also overlap.

Wellness tourism, in particular, is a broad concept; and its meaning depends on culture and geography in the EU. For example, in southern Europe, wellness tourism is connected to the seaside, the Mediterranean diet, and the slower pace of life. In Scandinavia, the focus of wellness is on outdoor activities like walking and swimming. As you can see, the definition of wellness tourism is diverse across the EU and serves as proof of the overlap between wellness and spa tourism.

All in all, the definition of health tourism is not really clear, making it difficult to include it in tourism statistics. That consequently makes it tricky to measure the importance, growth, and impact of health tourism on the overall tourism industry and economy.

However, we do know that the market for health tourism has grown exponentially in recent years and has led to new forms of health tourism like fertility tourism and dental tourism.

But how much is the industry growing?

Estimating the total size of the health tourism sector in the EU is difficult because of the limited and fragmented data out there; and also due to the overlapping definitions by different sources and countries.

Although we cannot accurately estimate the total size, the industry has grown substantially in the past decades as citizens of the EU travel from richer countries like the United Kingdom, Sweden, Germany, Netherlands, and Ireland to seek affordable healthcare in other countries, primarily in Eastern Europe.

It has been growing at 12-15 percent annually, according to research from PwC .

New call-to-action

Until quite recently, the top health destinations for Europeans were the United Kingdom, France, Germany, Italy, and Spain. However, since a new European Directive on Cross-Border Healthcare (which states that there should be free access to treatment in the community space for all EU citizens) in 2014, a new public-pay medical tourism market appeared. As a consequence, countries like Hungary, the Czech Republic, Romania, Poland, and Latvia have become popular destinations for Western Europeans. According to the Polish Association of Medical Tourism, 488,000 of foreign patients came to Poland for medical treatment in 2014.

Health tourism today makes up about 5 percent of the tourism industry in the EU28 and contributes to about 0.3 percent of the EU economy. It in fact has a higher domestic share than traditional tourism does. Therefore, increasing the share of health tourism may lessen the seasonality of tourism, increase labor quality and sustainability.

Perhaps that is one of the reasons countries like Turkey are implementing policies to fuel the health tourism market. In Turkey, which has become a premier and popular destination for medical tourists looking for less complicated treatments, the government offers discounts for patients flying with Turkish Airlines. The government is also planning to introduce tax-free healthcare zones for foreign patients.

More and more governments and companies are offering cheap flights, open borders, and inclusive packages to further fuel this segment of the tourism market.

Let’s take a look at some key statistics from a June 2009 survey that was conducted by the Medical Tourism Association (MTA) to understand why the industry is growing so fast and why it is important for key stakeholders to prioritize health tourism directives:

  • 64% of patients that traveled for care didn’t have health insurance
  • Almost 83% of patients traveled with a companion
  • 33% of patients traveled abroad for cosmetic surgery
  • Almost 90% of patients or their companions engaged in tourism activities
  • Nearly 80% of the demand for medical travel is driven by cost savings
  • Medical tourists spend between €6,083 and €12,885 per medical travel trip
  • 48% of respondents would be interested in engaging in medical tourism again at some point in the future
  • The cost of medical treatment and quality were the most important factors in their decision to travel abroad for treatment
  • Out of 49 participants that had previously traveled overseas for medical treatment, 36% used a medical tourism facilitator

The following statistics from Eurostat and IPK International  give us a general overview of the health tourism market in the EU:

  • The total volume of health tourism in the EU is around 56 million domestic arrivals and 5.1 million international arrivals in 2014. (That figure is for health tourism with the main objectives of wellness, spa, and medical treatments.)
  • Health tourism makes up 4.3 percent of all EU arrivals
  • Germany, France, and Sweden are the main players in the health tourism industry in the EU
  • Wellness tourism makes up 66-75 percent of EU health tourism
  • Health tourism brings in €46.9 billion in revenue which represents 4.6 percent of overall tourism revenues and 0.33 percent of EU28 GDP
  • More than 75 percent of EU health tourism revenues come from five countries: Sweden, France, Poland, Italy, and Germany
  • Market reports indicate stability and an increase in health tourism
  • Most medical health tourism clinics also serve local patients and use medical tourism as an additional revenue stream
  • Health tourism may have several benefits to the labor market and the environment

Patients choose cross-border treatments for various reasons: The possibility of having access to certain treatments that are not available in their countries, the lower cost and higher quality of the treatments; and the opportunity to have a holiday to rest and recover in the destination country. Patients may even spend more time in a hotel that they would at a hospital during their medical trip. Depending on the procedure, they often spend a couple of nights at a hospital and spend the rest of the time in a hotel for recovery or doing physical therapy.

A European Commission survey shows that a large proportion of EU citizens, about 49 percent, is willing to travel for health care. That may be due to the EU directive on healthcare, which says that Member States must reimburse their citizens at the value that the care they receive would cost at home (in order to protect the financial sustainability of national healthcare systems).

However, this gives certain disadvantages to patients from poorer countries with less developed healthcare systems. For example, healthcare services cost substantially less money in Croatia than in Sweden. So a Croatian patient would have to pay out of pocket for the substantial difference in the cost of treatment if they seek medical treatment. Whereas, a patient from a wealthier Member State can seek treatment in Croatia or anywhere else without paying much out of pocket for their care. That scenario is particularly beneficial for wealthier member states as they make economic gains if the treatment is less expensive abroad.

Health tourism is undoubtedly growing at an exponential rate and healthcare and tourism providers are picking up on it. So are governments. National policies in certain countries, like Turkey, are enhancing the growth of health tourism. That is evidence that health tourism is an important strategy within the overall tourism planning.

Medical Tourism is a rapidly growing global market sector in which patients travel to neighboring and even distant countries to receive medical treatment. Cross-border healthcare refers to a similar phenomenon within the European healthcare market.  Quality and price are the main driving forces in this new line of business. The medical tourism market has evolved from its initial days as a grassroots movement to the global phenomenon it is today, with ever increasing patient streams worldwide. ‍

This demands also more expertise and knowledge production in which academic institutions could play a supporting role in view of the patient and providers. ‍

This increasing number of patients who travel for health reasons has led to the foundation of the Medical Tourism Association and the European Medical Tourism Alliance, two organizations that were founded in order to explore and resolve all relevant issues concerning the new global health market.

As of now, there is not an adequate postgraduate qualification program available for those professionals working within this industrial sector, nor is there an academic community supporting this field with applied knowledge and tools.

Creating and providing a postgraduate qualification program is the aim of a new initiative developed by several leading European universities and cooperating companies, and will be supported by the two above mentioned organizations. The MBA presupposes the relation with governmental bodies, and as such has also attention for public health debates, in which the patient also takes a central place.

Joint European Educational Program

MBA for Health & Medical Tourism  / Cross-border Healthcare Management is designed as a joint European educational program aimed at guaranteeing the highest level of quality in the emerging business of medical tourism and cross-border healthcare. It places special emphasis on the entrepreneurial and innovation challenges faced by managers in all lines of business associated with this industry.

The program will be co-hosted by several leading universities in Europe and the US and will be offered on a block course basis for postgraduate professionals. A global approach is needed because of the complexity of the provision of international health services.  Health clusters  suppose an integrated management expertise which is embedded in various cooperating sector partners. ‍

Framework of the MBA Program

The program will be designed as a 60 ECTS study course organized in three semesters and held mostly in one week block courses. The third semester will include an extensive exchange program which must be completed abroad, as well as an internship which can be completed in the student’s home region or abroad.

Both the Medical Tourism Association Inc. and the European Medical Tourism Alliance (E.E.I.G.) have expressed their willingness to support the initial implementation, management and continuous development of the MBA program for Medical Tourism & Cross-border Healthcare Management.

Moreover, all related and relevant pre-existing professional associations (healthcare, tourism, business, etc.) on the regional, national and international levels will be introduced to the initiative and invited to participate in the development of the program.

Political Support

In order to strategically position the MBA program on respective national levels, contacts will be established or further extended with regulatory bodies and related ministries (Health, Tourism, Economy, etc.)

Several workshop formats have already been offered on various aspects of health and medical tourism so far to promote the program. The MBA Program is going to be gradually set up in 2009/2010 by educational initiatives.  

These block courses will be held at Deggendorf University (Five Day Excecutive Module, December 2009) and at Breda University (Four-day Cross-Disciplinary Masterclass International Health Services, January 2010), both with a unique and different approach.

These events will soon be followed by the first clinical blocks (UKE Hamburg, Barcelona) and certificate courses on health policy and legal issues (Bavarian Health Ministry, MCI Innsbruck, Bavarian Representation in Brussels)

There's only one country in the world where healthcare is free for everyone.

Free healthcare is a major reason that people move abroad, new research has revealed.

70 per cent of those surveyed in the US by Expatsi said they hoped to benefit from free healthcare in their target countries.

The high cost of healthcare in the US is likely a major contributing factor. Half of American adults say it's a challenge to keep up with healthcare costs and one in four say they or a family member has struggled to pay healthcare bills in the last 12 months, according to KFF.

Free medical care isn't the only reason people move abroad . 15 per cent of those polled by Gallup said they want to leave the country within the next year, up from 10 per cent a decade ago. The main reason they gave: "for adventure and personal growth".

Jen Barnett, co-founder of Expatsi, a company that's helped thousands of Americans move abroad , gives her advice on what she recommended soon-to-be expats focus on. "You need to find a place with the right weather, culture, laws, and language, but at the end of the day, it's all about vibes. We recommend scouting trips to find out what most feels like home."

What visas are would-be expats applying for?

Moving abroad isn't as difficult as it used to be. A laptop and a WiFi connection are often all that expat hopefuls think they'll need.

But being able to live and work legally in your target country is also essential if you don't want to run into problems with immigration or tax authorities.

Despite this, only 21 per cent of want-to-be expats surveyed said they are planning to apply for skilled work visas . More than 50 per cent planned to get a digital nomad visa or weren't sure which visa they would need.

Is healthcare free for expats in Europe?

Countries with universal or so-called 'free healthcare' do not necessarily offer it to everyone. 

Many countries offer it first to their own citizens. In some countries, residents, including expats, are also entitled to free healthcare. Tourists are less likely to benefit from a country's free health system.

As a foreigner, can you walk into a clinic or hospital in the UK and expect free healthcare? No. Outside of emergency treatment, 'medical tourists' need to pay for the health systems in those countries their citizens get for free.

If you have residency, you do gain access to Britain's National Health System (NHS). However, it's not completely free. Resident visas include an NHS contribution of £1,035 (€1,207) per year for full access to Britain's 'free' healthcare.

In most cases, this is provided by the French state’s social security system, known as the 'Assurance maladie'.

The state system, however, does not pay for 100% of all health costs. Most French residents also purchase top-up insurance from a company or a non-profit-making organisation, known as a 'complémentaire santé' or 'mutuelle'.

Bear in mind that if you are living in France but not employed there - like retirees or digital nomads - you may get an annual healthcare bill, known as the 'cotisation subsidiaire maladie'.

Is healthcare free in Spain?

Spain's Sistema Nacional de Salud (SNS) is similar to the UK's NHS. Citizens are covered for doctor appointments and treatment without the need to pay. Legal residents are also eligible for the system. 

However, like in the UK, don't expect to get treatments such as dental, eye care, or mental health professionals using the free health system. Private health insurance is necessary in those cases.

Is healthcare free in Italy?

When it comes to healthcare, Italy divides foreign residents into two categories: those who must register with the national health service ('Servizio sanitario nazionale', or SSN) on a 'mandatory' basis, and those who have the option of registering on a 'voluntary' basis.

Broadly, anyone employed in Italy falls into the 'mandatory' category, as well as those waiting for residency or citizenship. Those in the 'mandatory' category have the right to register with the SSN for free, while those who fall into the 'voluntary' category must pay to register.

To find out which category you fall into it's best to consult an Italian 'commercialista' (accountant).

Most of those who are registered with the SSN free usually have to pay for prescriptions and sometimes have to pay top-up fees at hospital appointments or procedures.

Is healthcare free in Germany?

Germany's healthcare system is regarded as one of the best in the world but it's not free.

Whether resident or citizen, health insurance is compulsory. Public health insurance, or 'Gesetzliche Krankenversicherung' (GKV), is the most common form of health insurance. If you opt for public insurance, you’ll pay a monthly premium to your health insurance provider, which is calculated according to your income. Your employer will also pay mandatory contributions to your insurance.

Private health insurance , or 'Private Krankenversicherung; (PKV), is the other form of health insurance in Germany.

If you’re self-employed, a small business owner, a freelancer, or work in the public service, you’ll most likely have to sign up for PKV, although there are still notable exceptions to this. Premiums for PKV are calculated based on a risk assessment done by the insurer, and generally vary based on your age and health.
