If the passport is lost / stolen, or its validity has expired, and there is a need for an urgent trip to Ukraine, we recommend issuing an identity card to return to Ukraine .

How to issue a new passport

Step 1 : Be a citizen of Ukraine permanently residing or temporarily staying in the consular district

Consular District Borders: Idaho, Arizona, Alaska, Wyoming, Washington, Hawaii, California, Colorado, Montana, Nevada, New Mexico, Oregon, Utah

Step 2 : Have the necessary documents according to the list. The list is contained in the corresponding section on this page

Step 3 : Arrive at the Honorary Consulate of Ukraine in Seattle and submit the documents in person. Reception of documents is carried out without prior registration in the order of the live queue according to the schedule of reception of visitors.

  • If a person is over 16 years old, the personal presence of this person is mandatory and the documents are submitted personally by this person.
  • If a person is under 16 years of age but over 12 years of age, the personal presence of this person is mandatory and the documents are submitted by one of the applicant’s parents
  • If the person is under 12 years of age, the personal presence of this person is not required and the documents are submitted by one of the applicant’s parents

Step 4 : Wait for the passport to be issued and delivered

Step 5 : After delivery of the issued passport, arrive at the Honorary Consulate of Ukraine in Seattle and personally receive the issued passport

The passport is issued at the Honorary Consulate of Ukraine in Seattle only in person to the applicant. A passport of a person under the age of 16 may be obtained by one of the parents (adoptive parents) of this person, provided that a passport or other identity document of the recipient is presented.

Upon receipt of a new passport, you must provide a previous passport.

If the passport was issued for the first time, or the previous passport is in the Honorary Consulate of Ukraine in Seattle, you must provide proof of identity. If the previous passport has been lost / stolen, a police report must be provided confirming the applicant’s complaint about the loss / theft of the passport and an identity document.

Necessary documents for a new passport

Document 1. Questionnaire. The application form is completed. Sample filling

Document 2. Photo

  • For a person under 12 years of age – 1 color printed photo of 10 x 15 cm with a clear image of the face on a white background (the face should occupy 80% of the photo)
  • For a person over 12 years of age – photography is carried out directly at the Honorary Consulate of Ukraine in Seattle, no additional fee is charged

Document 3. Passport

  • Passable foreign passport
  • Copies of passport pages with entries must also be attached

In case of absence of the passport, the following is submitted:

  • Police report – in case of loss or theft of the passport – the original police report stating that the applicant’s passport was lost / stolen. Additionally submitted: a copy of the lost passport (if available), a copy of the internal passport (if available).
  • Travel document of the child, or passport of one of the parents, where the person is registered – if available (if the person issues a passport for the first time).
  • Certificate of acquisition of Ukrainian citizenship – if a person was born abroad (in the US, etc.) and issues a passport for the first time
  • A copy of such a document must also be attached

Document 4. Parents’ permission to issue a passport

  • Only for a person under 16 years of age – The application is submitted by one of the parents who does not submit documents and does not fill out a questionnaire for a child’s passport and has Ukrainian citizenship. If you do not have the citizenship of Ukraine and have the citizenship of another state – a copy of the passport confirming the affiliation to foreign citizenship is provided.
  • In case of absence of one of the parents during the submission of documents, the relevant application is submitted notarized

Document 5. Birth certificate + copy – submitted only for a person under 16 years of age. Birth certificate issued in the USA (or in another foreign country) is submitted with a certified stamp “Apostille” and with a notarized translation into Ukrainian.

Document 6. Proof of stay in the United States

  • This can be one of the similar documents: “Green card”, Driver License, ID, valid visa, form I-94, certificate of employment, rental agreement, certificate of open a bank account, etc.
  • The last name and first name in such a document must match the last name and first name on the passport.

Document 7. Marriage certificate, divorce certificate, certificate of change of name or surname, or a relevant court decision – in the case of a passport for a new surname or name.

The relevant document must clearly indicate the transition from one surname / name to another. The document, issued by the competent authority of the United States, is submitted certified by “Apostille” and with a notarized translation into Ukrainian.

WARNING! You can issue a new passport with a new surname if:

  • the person has issued a permanent residence permit abroad
  • the person has issued an internal passport (passport of a citizen of Ukraine) with a new surname (a copy of such passport is attached)

Document 8. Consular fee + copy

$ 40 – for a passport for a person under 16 years of age $ 130 – for a passport for a person over 16 years of age

WARNING! The consular fee is submitted in person together with other documents! A copy of such money order must also be attached.

The consular fee is paid in the form of “ Money Order “. Cash, credit cards, checks, bank transfers are not accepted!

Type of issued passport

  • Passport of a citizen of Ukraine for travel abroad (Biometric passport)
  • Transliteration check
  • Validity of the issued passport: 10 years – for persons who have reached 16 years of age. 4 years – for persons under 16 years of age.

It is not possible to enter data on children in parents’ passports. Every citizen, regardless of age, can get a separate new passport.

The Honorary Consulate of Ukraine in Seattle does not transfer visas or other marks of foreign states to the passport.

Checking the status of the passport

To check the status of the passport, go to the link passport.mfa.gov.ua and enter the session ID, which was provided to you when applying for a passport.

  • Data sent for personalization – means that all checks have been carried out and your personal data has been sent to the printing plant for passport printing.
  • The document has been prepared – it means that the passport has already been printed, but has not yet been delivered to the Honorary Consulate of Ukraine in Seattle.
  • The document has arrived at ZSU – means that your passport has arrived at a foreign diplomatic mission, ie the Honorary Consulate of Ukraine in Seattle, and you can get it.

If you do not have a session ID, please fill out this form.

The term of registration (excluding the terms of delivery of the finished passport to the Honorary Consulate of Ukraine in Seattle from the Ministry of Foreign Affairs) is up to three months from the date of submission of documents.

Procedure for obtaining a manufactured passport

Upon receipt of a new passport, you must provide a previous passport. If the passport was issued for the first time, or the previous passport is in the Honorary Consulate of Ukraine in Seattle, you must provide proof of identity. If the previous passport has been lost / stolen, a police report must be provided confirming the applicant’s complaint about the loss / theft of the passport and an identity document.

Tensions have continued to heighten in Ukraine and the surrounding region. In view of the current situation, Singaporeans in Ukraine are advised to leave t he country  as soon as possible via commercial means while it is still possible to do so. There is no guarantee that MFA will be in a position to assist Singaporeans’ departure if conflict occurs. Singapore does not have a diplomatic mission in Ukraine. Singaporeans who are still in Ukraine should immediately e-register with MFA at  https://eregister.mfa.gov.sg  if they have not done so. In view of the escalating tensions and evolving situation  with regard to Ukraine and its surrounding region,  MFA advises Singaporeans to defer all travel to Ukraine. Singaporeans who are in Ukraine are strongly advised to remain vigilant and monitor local news closely. They should take necessary precautions for their personal safety, and eRegister with MFA immediately at  https://eregister.mfa.gov.sg  if they have not done so. Ministry of Health (MOH) Public Health Advisory Please refer to the  ICA website for updates on the latest precautionary measures implemented to further reduce the risk of importation of COVID-19 to Singapore. These include travel advisories and further restrictions on travellers coming into Singapore. 

On 28 September 2020, Ukraine lifted the temporary entry restrictions for foreigners and stateless persons traveling to or through Ukraine. Travellers are now permitted to cross the border into Ukraine provided they have an insurance policy certificate issued by a Ukraine-registered insurance company, or a foreign insurance company with an office in Ukraine or a contractual partnership with one of the Ukraine registered insurance companies. The insurance policy must cover the potential costs of COVID-19 treatments, observation, and be valid for the period of the intended stay in Ukraine. Visitors entering Ukraine from countries with high numbers of COVID-19 cases will be required to self-isolate on arrival. The list of countries considered to have high-numbers of COVID-19 cases can be found at https://www.moz.gov.ua . Persons who self-isolate after entering Ukraine must also install and activate the appropriate mobile phone app. Those who cannot do so will be subject to observation. Visitors wishing to enter Ukraine can now apply for an e-visa, which can be issued for a single or a double entry into Ukraine, and allow holders to stay for up to 30 days where the purpose of the trip is business / private visit / tourism / medical treatment / cultural, scientific, educational or sports activities / foreign mass media staff visit. Singaporeans travelling to or already in Ukraine should take the necessary precautions for their personal safety, heed the instructions of the local authorities, and closely monitor the local news. Singaporeans should remain vigilant and exercise increased caution such as avoiding public gatherings and crowded places. They should also purchase comprehensive travel insurance and be familiar with the terms and coverage.

Entry and Exit

Singaporeans can obtain electronic visas (e-visa) which are valid for 30 days.

Singaporeans can also obtain visa-on-arrival for a stay of up to 15 days at Boryspil and Odesa international airports if they meet the following criteria:

- diplomatic / official passport holders visiting at the invitation of Ukraine’s state authorities;

- visitors who intend to enter Ukraine in order to render assistance in eliminating the aftermath of an emergency/accident in Ukraine;

- visitors who intend to enter Ukraine for urgent medical treatment or participation in the burial of a close relative in Ukraine; or

- pilots and other aircraft crew members carrying out international flights (with pilot’s license or a Flight Crew Member Certificate issued in line with the  Convention on International Civil Aviation ).

As immigration requirements can change at short notice, travellers are advised to check with their travel agent or the Embassy of Ukraine in Singapore ( http://singapore.mfa.gov.ua/en ) for the latest information.

Safety and Security

Travellers are advised to exercise a high degree of caution in Ukraine due to the unpredictable security situation across the country and threat of terrorist attacks. Petty and violent crime, including robberies, pickpocketing and drink-spiking, occur regularly in Ukraine. Be aware of your surroundings and take measures to protect your personal security. Avoid carrying large amounts of cash or wearing jewellery. Avoid travelling alone, especially after dark. Protests do occasionally take place. Exercise caution, avoid affected areas, monitor developments through the local news, and heed the instructions of the local authorities. Be vigilant when using ATMs in public places. Check for card skimming devices at ATMs as there have been instances of credit card and ATM fraud. Ensure that credit card charges are accurate before signing, keep all receipts, and verify against your credit card statements.

It is a customs regulation to declare cash and jewellery upon arrival in Ukraine. It is a requirement to carry identification at all times. Smoking and consumption of alcohol in public places are prohibited. It is illegal to take photos of or near military and government establishments. Driving: Local driving conditions are often poor. Streets are dimly-lit, and speed limits, traffic lights and road signs often ignored. Exercise extra caution while driving.

Additional Information

There are risks of measles infection and tick-borne encephalitis, a viral disease that affects the central nervous system. See your doctor for advice on appropriate vaccinations before you travel. Take precautions against insect bites. Purchase comprehensive travel insurance before departure. Chernobyl is open to tourism but is still radioactive due to the nuclear accident. Adhere to all safety instructions from your tour operator and local authorities. Bush and forest fires may break out in eastern and southern Ukraine. Monitor developments through the local news and heed the instructions of the local authorities.

Overseas Travel – Be Informed & Be Safe [Updated on 5 February 2024]

Singaporeans planning overseas travel are reminded to take the necessary precautions, including being prepared to deal with accidents, natural disasters or terrorist attacks. Singaporeans are also reminded to be familiar with your destination’s local laws, customs, and COVID-19 regulations.

Demonstrations do occur in major cities across the world. Such demonstrations can sometimes escalate into violence. It is important for Singaporeans to keep abreast of local news, avoid any protests or demonstrations and heed the advice of the local authorities.

When participating in outdoor leisure activities overseas, Singaporeans should be mindful that certain sporting activities, especially in open seas, may carry risks. Besides ensuring that one has the physical competencies and appropriate condition to undertake the activity, every effort should be made to ascertain if the trip organiser or guide is reliable and competent, and that appropriate safety and contingency plans are in place. When in doubt, Singaporeans should consult the relevant professional bodies or sporting associations for specific advice.

  • Purchase comprehensive travel insurance and be familiar with the terms and coverage.
  • Equip yourself with research about your destination’s entry requirements, current situation, local laws and customs.

While travelling

  • Always take care of your personal safety, remain vigilant and monitor local weather news, advisories, and security developments.
  • Exercise caution around large gatherings and avoid locations known for demonstrations or disturbances.
  • Be prepared for possible delays and last-minute changes in travel plans especially during unforeseen events such as natural disasters, social unrest or terror attacks.
  • Stay connected with your friends and family. Inform them of your whereabouts and provide them with your overseas contact details.

Advisory: Email Scams

There have been reports of individuals receiving scam emails/messages purportedly sent from friends in distress overseas. These emails/messages typically originate from an email address/social media known to the receiver bearing claims of the sender getting into trouble overseas and urgently requesting financial assistance. The sender would also claim to have approached a Singapore Embassy/Consulate and the local Police for help to no avail.

MFA takes the safety of all Singaporeans very seriously.  Singaporeans in distress approaching our Overseas Missions for assistance will be rendered with all necessary consular assistance.  If you receive such emails/messages from purported friends seeking funds transfers, we strongly advise you to call them first to verify the authenticity of the emails/messages before responding to their request.  It is also not advisable to give out any personal information such as NRIC/passport numbers, address, telephone number, etc.  Any form of reply, even one of non-interest, could result in more unsolicited emails.  Members of the public who suspect that they have fallen prey to such scams should report the matter to the Police immediately.  Should Singaporeans abroad require consular assistance, they can contact the nearest Singapore Overseas  Mission  or call the Ministry of Foreign Affairs 24-hr Duty Office at +65 6379 8800/+65 6379 8855.

The U.S. Department of Agriculture, Food and Drug Administration, Centers for Disease Control and Prevention, and State veterinary and public health officials are investigating an illness among dairy cows.

As we learn more about this evolving situation, APHIS will continue to provide confirmatory testing for samples from livestock as well as guidance for producers, veterinarians, and state animal health officials. As the situation changes, we will continue to post updated information at the links below. 

Clarification Regarding Movement of Cattle Through Markets or Auctions to Slaughter, Under the April 24 Federal Order

USDA APHIS continues to receive questions related to the movement of lactating dairy cattle to sale barns and auction markets under the Federal Order and associated guidance , effective on April 29.

This message is to clarify that the Federal Order does not apply to the intrastate movement of a lactating dairy cow to a sale barn. Subsequent interstate movement for a lactating dairy cow from a sale barn directly to a slaughter facility requires only a Certificate of Veterinary Inspection (CVI) stating that the animal is clinically healthy; no testing is necessary. 

This clarifies the requirements for states where direct to slaughter movements are limited but cattle movements flow through sale barns and auctions for consolidation and movement as a "lot" to slaughter out of state. We are announcing this clarification over the weekend to ensure small farms have the guidance necessary to continue to move cull cattle and limit animal welfare issues.

Confirmed Cases of HPAI in Domestic Livestock

Testing at National Animal Health Laboratory Network (NAHLN) and National Veterinary Services Laboratories (NVSL) is complex; the number of tests conducted is not equivalent to animals tested. The testing figure may include multiple samples collected from a single animal, samples tested independently or pooled, samples tested more than once, and testing for purposes such as on farm research studies to continue to learn more about this virus in livestock. As such, the number of affected herds provides a much more accurate picture of the evolving situation than the number of tests conducted.

On April 24, USDA announced a Federal Order that requires mandatory testing prior to the interstate movement of lactating dairy cattle and mandatory reporting of positive influenza A test results in livestock. This testing is critical to increasing the information available to the USDA and to limiting potential virus spread through livestock movement. (Testing information updated weekly on Wednesdays by 4 pm ET)

  • Total PCR tests run at NAHLN and NVSL (since April 2024): >15,000 * APHIS identified a computational error in how the number of PCR tests conducted at the National Veterinary Services Laboratory (NVSL) and the National Animal Health Laboratory Network (NAHLN) laboratories are summed.  As a result, APHIS reported an inaccurate number of total tests on the website.  Additionally, APHIS also realized that testing information from NVSL and the NAHLN laboratories were reported based on different start dates.  To address both issues, APHIS realigned the data to correct the computational error and use a consistent start date for reporting-the effective date of the Federal Order, April 29, 2024.
