Telehealth is the use of telecommunications and information technology to provide access to health assessment, diagnosis, intervention, consultation, supervision and information across distance. At one time, telehealth in Medicaid had been referred to as telemedicine.

Telehealth seeks to improve a patient's health by permitting two-way, real-time interactive communication between the patient and the physician or practitioner at the distant site. This  communication often requires  the use of interactive telecommunications equipment that can include both audio and video components, but can also be conducted via audio-only, as states deem appropriate.

Telehealth includes such technologies as telephones, electronic mail systems, and remote patient monitoring devices, which are used to collect and transmit patient data for monitoring and interpretation.

State Telehealth Flexibilities

For most Medicaid benefits, federal Medicaid law and regulations do not specifically address telehealth delivery methods or the criteria for implementation of telehealth.  As a result, states have broad flexibility in designing the parameters of telehealth delivery methods to furnish services. However, underlying services must continue to meet the requirements of the overarching provisions in Title XIX of the Social Security Act (the Act), regulations, the federal policy framework of the covered Medicaid benefit, and the parameters of the state’s CMS-approved Medicaid state plan or a subsequent state plan amendment (SPA). Where Medicaid law or regulations set forth telehealth delivery requirements for specific benefits, those requirements must be observed. For example, the Community First Choice (CFC) Option at 1915(k) has general requirements for using telehealth for performing the assessment of need (§441.535), but does not have requirements for other CFC activities that could be performed using telehealth.

CMS Approach to Reviewing Telehealth SPAs

Unless required by regulation or policy, states are not required to submit a (separate) SPA for coverage or reimbursement of Medicaid coverable services delivered through telehealth if they decide to reimburse for services delivered through telehealth in the same way/amount that they pay for face-to-face services.

States must submit a (separate) reimbursement (attachment 4.19B) SPA if they want to provide reimbursement for services or components of services delivered through telehealth differently than is currently being reimbursed for face-to-face services.

States may submit a coverage SPA to better describe the services they choose to cover through telehealth, such as which providers/practitioners are identified by the state to use telehealth to deliver services; where it is provided; how it is provided, etc. In this case, and in order to avoid unnecessary SPA submissions, it is recommended that a brief description of the framework of telehealth may be placed in an introductory section of the state plan, e.g., Section 3 – Services: General Provisions 3.1 Amount, Duration and Scope of Services, and then a reference made to coverage through telehealth in the applicable benefit sections of the state plan. For example, in the physician section it might say that dermatology services can be delivered via telehealth provided all state requirements related to telehealth as described in the state plan are otherwise met.

There is no question that telemedicine, also known as virtual care, has expanded and improved patient access to health care.

At Scripps, many health care needs — from wellness visits to follow-up care — can now be met through a video visit with a Scripps provider.

Video visits have been welcomed by many patients, leading doctors to believe that they are here to stay.  Virtual care offerings at Scripps also include E-visits and Symptom Checker .

“Video visits won’t replace every visit, but where it makes sense, especially for care that doesn’t require a physical exam in person, they will continue to be a secure and convenient option for patients,” says David Wetherhold, MD , a Scripps Clinic internal medicine physician and Scripps chief medical information officer for ambulatory care.

Video visits in real time, on your time

Many Scripps primary care providers and specialists offer video visits. They are available by appointment only, which can be done by contacting your physician’s office directly or scheduling online.

Scripps HealthExpress offers video visits for minor illnesses and injuries seven days a week. Video visits with HealthExpress providers can be scheduled online through the MyScripps patient portal or can be conducted “on demand.” 

Setting up is simple

Scripps video visits are conducted through the MyScripps patient portal. You can use any internet-connected device (smartphone, computer or tablet) with a working camera and microphone. You should also take a few minutes to make sure your device has the latest operating system update.

Health tools, such as a blood pressure cuff or thermometer, aren’t required, but they do come in handy, since some conditions can be managed or monitored remotely. 

Scripps is also part of the Apple Health app platform, meaning Apple Watches and other compatible devices can auto-upload data like your blood pressure, pulse and glucose levels to the MyScripps app , where your physician can easily access it.

12 tips for planning your video visit

Whether this is your first time having a medical appointment by video or you’re a veteran, there are steps you can take to make sure your visit goes smoothly. Watch our short video tutorials and read these tips from Dr. Wetherhold:

1. Check Wi-Fi

Call from a quiet place that has good phone and Wi-Fi service. Remember to turn off background noise from the television or radio.

2. Find a private place

Set up in a private area, so you can discuss personal medical information.

3. Prepare your questions

Write down questions you have in advance for your provider before the video visitation.

4. Get good light

Make sure there is plenty of light. Face a window or light if possible. You want to avoid sitting with your back in front of a window or other bright light as this will make it difficult for the provider to see you.

5. Hold your device horizontally

If using a mobile device, orient your phone or tablet horizontally and not vertically.

6. Provide good view of yourself

Your provider wants to see you! Make sure your face and upper body are in the video and not just the top of your head.

7. Consider getting camera help

You may be asked to move the camera to assist in evaluating a problem. Consider if you will want help in holding the camera if you are asked to perform physical exam maneuvers.

8. Test your device

 Log in to your online patient portal and test your device before the visit to troubleshoot any technical issues. Ensure that you are not on mute and that the microphone and camera on your device are enabled. At Scripps, the online patient portal is MyScripps .

9. Check in early

Start your visit check-in process at least 15 minutes prior to the scheduled time.

10. Silence your phone

If you are using your mobile phone, don’t be on a phone call when the visit begins.

11. Tap to hear better

If you can’t hear the audio, tap the screen and then press the microphone button twice.

12. Be ready to take notes

Have pen and paper to write down information.

To com­plete a video vis­it, you must have an active MyChart account. If you are new to Duly Health and Care or have not pre­vi­ous­ly acti­vat­ed your MyChart account, request an acti­va­tion code online here or by call­ing the MyChart Help Desk at 1−855−269−2427.

How to sched­ule, pre­pare for and launch a video visit:

In this tuto­r­i­al, we demon­strate how to sched­ule all the way through launch­ing a video vis­it using the MyChart app on a mobile device. 

Get­ting the most from your video visit 

While you can address many com­mon ill­ness­es, pre­scrip­tion refills or even com­plete an annu­al phys­i­cal as you would dur­ing an in-clin­ic appoint­ment, video vis­its are slight­ly dif­fer­ent than in-clin­ic appoint­ments. You will not speak to a nurse or med­ical assis­tant before see­ing your doc­tor as you would in-clin­ic. To get the most out of your face-to-face time with your provider, we rec­om­mend writ­ing down a pri­or­i­tized list of ques­tions or con­cerns that you’d like to dis­cuss. Here are a few thoughts to jot down: 

Annu­al Phys­i­cals or Refills

  • Any changes in your health over the last year
  • Your diet and exer­cise habits
  • Fam­i­ly history
  • Med­ica­tions (pre­scrip­tion or over the counter)
  • Surg­eries.

Ill­ness­es or New Problems

  • Your tem­per­a­ture just before your visit
  • Symp­toms you’re experiencing
  • How long you’ve had each symptom

While a major­i­ty of the time video vis­its are com­plet­ed with­out a hitch, occa­sion­al­ly some issues do pop up. Here are some of the most com­mon issues and tips on troubleshooting. 

Access­ing MyChart

  • To com­plete a video vis­it you must have a MyChart account. With­out an account you will not be able to com­plete your video vis­it. Allow for plen­ty of time before your sched­uled vis­it to set up your account and request an acti­va­tion code online here or by call­ing the MyChart Help Desk at 1−855−269−2427.
  • If you can­not remem­ber your MyChart account user­name or pass­word, click For­got User­name or For­got Pass­word on the MyChart home­page to receive an email with your user­name or reset your password.
  • You can access your MyChart account on your desk­top com­put­er or a mobile brows­er at https://​mychart​.dupagemd​.com or through the MyChart iOS or Android app. We high­ly rec­om­mend log­ging in and con­firm­ing your account details pri­or to sched­ul­ing any video visit.

  • You must com­plete the full eCheck-in process, answer­ing screen­ing ques­tions and con­firm­ing the details of your account before you will be able to begin the video vis­it. You will have access to com­plete eCheck-in sev­er­al days pri­or to your appointment. 
  • Be sure to test your cam­era and micro­phone through your MyChart account pri­or to the vis­it to ensure mak­ing the most of your time. 

Start­ing the Visit

  • Log in to your MyChart account, click on Appoint­ments on the mobile app or select Appoint­ments and Vis­its under Vis­its in the main nav­i­ga­tion to see your upcom­ing appoint­ments. Select Begin Vis­it on the appoint­ment. Your appoint­ment will open in a new web brows­er on your device.
  • If you have com­plet­ed the eCheck-in process and the Begin Vis­it but­ton on the appoint­ment is still not avail­able, please close the app and log in again. 
  • You can con­nect to the vis­it 10 min­utes pri­or to the sched­uled appoint­ment time. If you begin the vis­it soon­er, you will receive an error and can con­nect again with­in 10 min­utes of the vis­it time. 
  • E‑Visits are a sep­a­rate ser­vice offer­ing from video vis­its. Do not click on Begin E‑Visit at the time of your appointment. 

Audio & Video Quality

  • Make sure the vol­ume is turned up on your device and not set to vibrate on a mobile device. 
  • Using head­phones with a micro­phone can help improve the sound quality. 
  • Make sure all oth­er appli­ca­tions on your device are closed and not using your net­work’s band­width. Avoid stream­ing media (Net­flix, YouTube, etc.) on your Wi-Fi or data net­work at the same time as your video visit.

An aspiring medical student was left brain dead last month when his friends pushed him into a Louisiana lake knowing he couldn’t swim — and then looked on while doing nothing to save the drowning man, according to a report.

Shocking video shows the group casually peering into the water moments after they shoved Christopher Gilbert off the dock at Lake D’Arbonne in Farmerville on April 14.

At least one woman can be seen slowly entering the water before abandoning the rescue mission.

It was another 10 minutes before a patron at a nearby restaurant intervened and yanked Gilbert back to land — just in the nick of time.

Christopher Gilbert was hospitalized after being submerged underwater for nearly 5 minutes after a group of people allegedly pushed him off a dock into a lake on April 14.

The 26-year-old was brain dead and the rest of his organs were starting to fail by the time he arrived at a local hospital, his mother Yolanda George told KSLA.

He spent 72 hours on a ventilator and an ECMO machine, which George described as “dialysis for the lungs.”

“I was devastated. I felt like my life had ended in that moment. My son is aspiring to be a medical doctor, my son is going to be a medical doctor. He got his masters last year in biological science. He’s preparing for medical school so for this to have happened to him … I was just devastated,” George said.

According to police, his so-called friends initially tried to pass off the near-death as “horse-play.”

Shocking video shows the group casually peering into the water moments after they shoved Gilbert off the dock at Lake D’Arbonne in Farmerville on April 14.

They even told the lie to George in the moments after he was rescued: “A friend of his called and she was hysterical crying. She told me that Chris has fell into the lake and he had been underwater for 10 minutes or so,” the mother said.

Eventually, a white female around the same age as Gilbert admitted to pushing him into the lake.

None of the friends, however, admitted to knowing he couldn’t swim — a claim Gilbert’s family finds hard to believe.

“To add insult to injury, after pushing him, no one from the ‘friend group’ attempted to go in after him. Instead, two brave bystanders, who have no relationship to Chris, heard commotion and retrieved his body from the lake,” personal injury attorney Claudia Payne, representing George, wrote in a report obtained by the Lincoln Parish Journal.

Fortunately, Gilbert has made strides since the ruthless attack.

He is responding cognitively, but cannot speak, according to George. He still relies on life support and his lungs are said to be at 20% capacity.

The 26-year-old was brain dead and the rest of his organs were starting to fail by the time he arrived at a local hospital.

The family is also demanding that an arrest be made in the tragic incident, with Payne saying the friends displayed criminal intent by pushing him into the lake.

“Why would you push my son in the lake knowing he couldn’t swim?” asked George.

Optum Rx is an affiliate of UnitedHealthcare Insurance Company. Optum Home Delivery is a service of Optum Rx pharmacy. You are not required to use Optum Home Delivery to obtain at least a 3-month supply of your maintenance medication. If you have not used Optum Home Delivery, you must approve the first prescription order sent directly from your doctor to Optum Rx before it can be filled. New prescriptions from the pharmacy should arrive within ten business days from the date the completed order is received, and refill orders should arrive in about seven business days. News | Senior living: Medicare’s push to improve chronic care attracts businesses, but not many doctors

Carrie Lester looks forward to the phone call every Thursday from her doctors’ medical assistant, who asks how she’s doing and if she needs prescription refills. The assistant counsels her on dealing with anxiety and her other health issues.

Lester credits the chats for keeping her out of the hospital and reducing the need for clinic visits to manage chronic conditions, including depression, fibromyalgia and hypertension.

“Just knowing someone is going to check on me is comforting,” said Lester, 73, who lives with her dogs, Sophie and Dolly, in Independence, Kansas.

At least two-thirds of Medicare enrollees have two or more chronic health conditions, federal data shows. That makes them eligible for a federal program that, since 2015, has rewarded doctors for doing more to manage their health outside of office visits.

But while early research found the service, called Chronic Care Management, reduced emergency room and in-patient hospital visits and lowered total health spending, uptake has been sluggish.

Federal data from 2019 shows just 4% of potentially eligible enrollees participated in the program, a figure that appears to have held steady through 2023, according to a Mathematica analysis. About 12,000 physicians billed Medicare under the CCM mantle in 2021, according to the latest Medicare data analyzed by KFF Health News. (The Medicare data includes doctors who have annually billed CCM at least a dozen times.)

By comparison, federal data shows about 1 million providers participate in Medicare.

Even as the strategy has largely failed to live up to its potential, thousands of physicians have boosted their annual pay by participating, and auxiliary for-profit businesses have sprung up to help doctors take advantage of the program. The federal data showed about 4,500 physicians received at least $100,000 each in CCM pay in 2021.

Through the CCM program, Medicare pays to develop a patient care plan, coordinate treatment with specialists and regularly check in with beneficiaries. Medicare pays doctors a monthly average of $62 per patient, for 20 minutes of work with each, according to companies in the business.

Without the program, providers often have little incentive to spend time coordinating care because they can’t bill Medicare for such services.

Health policy experts say a host of factors limit participation in the program. Chief among them is that it requires both doctors and patients to opt in. Doctors may not have the capacity to regularly monitor patients outside office visits. Some also worry about meeting the strict Medicare documentation requirements for reimbursement and are reluctant to ask patients to join a program that may require a monthly copayment if they don’t have a supplemental policy.

“This program had potential to have a big impact,” said Kenneth Thorpe, an Emory University health policy expert on chronic diseases. “But I knew it was never going to work from the start because it was put together wrong.”

He said most doctors’ offices are not set up for monitoring patients at home. “This is very time-intensive and not something physicians are used to doing or have time to do,” Thorpe said.

For patients, the CCM program is intended to expand the type of care offered in traditional, fee-for-service Medicare to match benefits that — at least in theory — they may get through Medicare Advantage, which is administered by private insurers.

But the CCM program is open to both Medicare and Medicare Advantage beneficiaries.

The program was also intended to boost pay to primary care doctors and other physicians who are paid significantly less by Medicare than specialists, said Mark Miller, a former executive director of the Medicare Payment Advisory Commission, which advises Congress. He’s currently an executive vice president of Arnold Ventures, a philanthropic organization focused on health policy. (The organization has also provided funding for KFF Health News.)

Despite the allure of extra money, some physicians have been put off by the program’s upfront costs.

“It may seem like easy money for a physician practice, but it is not,” said Namirah Jamshed, a physician at UT Southwestern Medical Center in Dallas.

The CCM program was cumbersome to implement, Jamshed said, because her practice was not used to documenting time spent with patients outside the office, a challenge that included finding a way to integrate the data into electronic health records. Another challenge was hiring staff to handle patient calls before her practice started getting reimbursed by the program.

Only about 10% of the practice’s Medicare patients are enrolled in CCM, she said.

Jamshed said her practice has been approached by private companies looking to do the work, but the practice demurred out of concerns about sharing patients’ health information and the cost of retaining the companies. Those companies can take more than half of what Medicare pays doctors for their CCM work.

Physician Jennifer Bacani McKenney, who runs a family medicine practice with her father in Fredonia, Kansas — where Carrie Lester is a patient — said the CCM program has worked well.

She said having a system to keep in touch with patients at least once a month has reduced their use of emergency rooms — including for some who were prone to visits for nonemergency reasons, such as running out of medication or even feeling lonely. The CCM funding enables the practice’s medical assistant to call patients regularly to check in, something it could not afford before.

For a small practice, McKenney said, having a staffer who can generate extra revenue makes a big difference.

While she estimates about 90% of their patients would qualify for the program, only about 20% are enrolled. One reason is that not everyone needs or wants the calls, she said.

While the program has captured interest among internists and family medicine doctors, it has also paid out hundreds of thousands of dollars to specialists, such as those in cardiology, urology and gastroenterology, the KFF Health News analysis found. Primary care doctors are often seen as the ones who coordinate patient care, making the payments to specialists notable.

A federally funded study by Mathematica in 2017 found the CCM program saves Medicare $74 per patient per month, or $888 per patient per year — due mostly to a decreased need for hospital care.

The study quoted providers who were unhappy with attempts to outsource CCM work.

“Third-party companies out there turn this into a racket,” the study cited one physician as saying, noting companies employ nurses who don’t know patients.

Nancy McCall, a Mathematica researcher who co-authored the 2017 study, said doctors are not the only resistance point.

“Patients may not want to be bothered or asked if they are exercising or losing weight or watching their salt intake,” she said.

Still, some physician groups say it’s convenient to outsource the program.

UnityPoint Health, a large integrated health system based in Iowa, tried doing chronic care management on its own, but found it administratively burdensome, said Dawn Welling, the UnityPoint Clinic’s chief nursing officer.

For the past year, it has contracted with a Miami company, HealthSnap, to enroll patients, have its nurses make check-in calls each month, and help with billing. HealthSnap helps manage care for more than 16,000 of UnityHealth’s Medicare patients — a small fraction of its Medicare patients, which includes those enrolled in Medicare Advantage.

Some doctors were anxious about sharing patient records and viewed the program as a sign they weren’t doing enough for patients, Welling said. But, she said, the program has been helpful, particularly to many enrollees who are isolated and need help changing their diet and other behaviors to improve health.

“These are patients who call the clinic regularly and have needs, but not always clinical needs,” Welling said.

Samson Magid, CEO of HealthSnap, said more doctors have started participating in the CCM program since Medicare increased pay in 2022 for 20 minutes of work, to $62 from $41, and added billing codes for additional time.

To help ensure patients pick up the phone, caller ID shows HealthSnap calls as coming from their doctor’s office, not from wherever the company’s nurse might be located. The company also hires nurses from different regions so they may speak with dialects similar to those of the patients they work with, Magid said.

He said some enrollees have been in the program for three years and many could stay enrolled for life — which means they can bill patients and Medicare long-term.
