The End Date of your trip can not occur before the Start Date.

Traveler reimbursement is based on the location of the work activities and not the accommodations, unless lodging is not available at the work activity, then the agency may authorize the rate where lodging is obtained.

Unless otherwise specified, the per diem locality is defined as "all locations within, or entirely surrounded by, the corporate limits of the key city, including independent entities located within those boundaries."

Per diem localities with county definitions shall include "all locations within, or entirely surrounded by, the corporate limits of the key city as well as the boundaries of the listed counties, including independent entities located within the boundaries of the key city and the listed counties (unless otherwise listed separately)."

When a military installation or Government - related facility(whether or not specifically named) is located partially within more than one city or county boundary, the applicable per diem rate for the entire installation or facility is the higher of the rates which apply to the cities and / or counties, even though part(s) of such activities may be located outside the defined per diem locality.

I-131, Application for Travel Document

Alert:  Beginning July 1, 2022, we will issue a new travel authorization document to Temporary Protected Status (TPS) beneficiaries: Form I-512T, Authorization for Travel by a Noncitizen to the United States, at our discretion if we find the beneficiary merits this authorization. We will no longer issue advance parole documents as evidence of our prior authorization for a TPS beneficiary to be permitted to reenter the United States if the beneficiary travels outside the United States.

Beginning July 1, 2022, we will issue a new travel authorization document to Temporary Protected Status (TPS) beneficiaries: Form I-512T, Authorization for Travel by a Noncitizen to the United States, at our discretion if we find the beneficiary merits this authorization. We will no longer issue advance parole documents as evidence of our prior authorization for a TPS beneficiary to be permitted to reenter the United States if the beneficiary travels outside the United States.

If you are a TPS beneficiary with an existing, unexpired advance parole document, you may continue to travel and seek reentry to the United States after a trip outside the United States through the period of validity printed on your advance parole document.

If you are a TPS beneficiary applying for a new travel authorization document, you should continue to use Form I-131, Application for Travel Document. If you have a pending Form I-131, you do not need to file a new application.

We will continue to issue advance parole documents to noncitizens with pending initial applications for TPS (Form I-821).

TPS beneficiaries and individuals with pending initial TPS applications should carefully read the Form I-131 Instructions which contain warnings about certain risks an individual may face if they are outside of the United States while USCIS is considering their TPS reregistration or initial application, such as missing important request for evidence or other notices or being denied TPS while outside the United States.

ALERT: Court decisions regarding DACA.

On Sept. 13, 2023, the U.S. District Court for the Southern District of Texas issued a decision finding the DACA Final Rule unlawful and expanding the original July 16, 2021, injunction and order of vacatur to cover the Final Rule. However, the court maintained a partial stay of the order for “all DACA recipients who received their initial DACA status prior to July 16, 2021.” See the Memorandum and Order (PDF, 1.35 MB)  and Supplemental Order of Injunction (PDF, 72.53 KB) .

Accordingly, current grants of DACA and related Employment Authorization Documents (EADs) remain valid until they expire, unless individually terminated.  In accordance with this decision, USCIS will continue to accept and process DACA renewal requests and accompanying applications for employment authorization under the DACA regulations at 8 CFR 236.22 and 236.23, as it has since October 31, 2022. We will also continue to accept initial DACA requests, but in accordance with the District Court’s order, we will not process initial DACA requests.

Current valid grants of DACA and related EADs will continue to be recognized as valid under the Final Rule. This means that individuals with DACA and related EADs do not have to submit a request for DACA or employment authorization until the appropriate time to seek renewal.

Use this form to apply for a reentry permit, refugee travel document, TPS travel authorization document, advance parole document (including parole into the United States for urgent humanitarian reasons or significant public benefit), or advance permission to travel for Commonwealth of the Northern Mariana Islands (CNMI) long-term residents.

If you file this form to request an Advance Parole Document authorizing you to seek parole in the United States when you return to the United States after temporary travel abroad, and you depart the United States before we issue your Advance Parole Document, we will consider your Form I-131 abandoned unless you were previously issued an Advance Parole Document that remains valid for the entire time you are outside the United States.

If you file this form to request an advance permission to travel for CNMI long-term residents document, and you leave the CNMI without having an advance permission to travel document, your status will automatically terminate.

You can pay the fee with a money order, personal check, or cashier’s check, or pay by credit card or debit card using Form G-1450, Authorization for Credit Card Transactions . If you pay by check, you must make your check payable to the U.S. Department of Homeland Security.

When you send a payment, you agree to pay for a government service. Filing fees are final and non-refundable, regardless of any action we take on your application, petition, or request, or if you withdraw your request.  If you pay by credit card or debit card, you cannot later dispute the payment. If you are submitting multiple forms, pay each filing fee separately. We are transitioning to electronically processing immigration benefit requests, which requires us to use multiple systems to process your package. We may reject your entire package if you submit a single, combined payment for multiple forms.

Payment if you file at a field office: You cannot pay fees with a money order or cashier’s check when filing at a field office. You can only pay with a personal check, debit card, credit card, or reloadable prepaid credit or debit card.

You do not need to pay an additional fee for Form I-131 if:

  • You are filing Form I-131 Application Type B or D;
  • You filed a Form I-485 with a fee on or after July 30, 2007, and before April 1, 2024; and
  • Your Form I-485 is still pending.

For refugee travel document applications filed from outside of the United States, you must pay the applicable fee(s) to the cashier at the USCIS overseas office or U.S. embassy or consulate with jurisdiction over your location. Please see the website of the appropriate  embassy or consulate  to confirm acceptable forms of payment. Include the fee receipt from the U.S. embassy or consulate when you file your application package.

Please do not mail cash, personal checks or traveler’s checks. If you do not include a fee receipt with your filing, we will reject your application.

Please do not submit this checklist with your Form I-131. The checklist is an optional tool to use as you prepare your form, but does not replace statutory, regulatory, and form instruction requirements. We recommend that you review these requirements before completing and submitting your form. Do not send original documents unless specifically requested in the form instructions or applicable regulations.

If you submit any documents (copies or original documents, if requested) in a foreign language, you must include a full English translation along with a certification from the translator verifying that the translation is complete and accurate, and that they are competent to translate from the foreign language into English.

Request for Travel Authorization (RTA)

When to complete an RTA: for all employee, prospective employee, and student travel

Completing a Request for Travel Authorization (RTA) is the first thing you should do when you start planning a trip for employees, prospective employees, or students using a centrally billed airfare or requesting reimbursement for travel on university business.

  • Automated RTA — You can complete the electronic VE5 document using DEFINE. RTA numbers are automatically assigned once the document receives initial approval. Travel Management Services recommends using the automated RTA whenever possible.

Student Travel Policy for International Locations (STPIL)

According to Appendix N of the University’s General Catalog , all students traveling internationally on University-related business are required to register their travel with Texas Global Risk and Safety. This is to ensure that essential emergency details are collected and that all students are enrolled in UT’s Overseas Insurance policy . This registration process is separate from and in addition to the Request for Travel Authorization (RTA) process required for reimbursement. However, failure to properly register travel can result in disciplinary action and the inability to receive reimbursement for travel expenses.

ESTA is an automated system that determines the eligibility of visitors to travel to the United States under the Visa Waiver Program (VWP) . Authorization via ESTA does not determine whether a traveler is admissible to the United States. U.S. Customs and Border Protection officers determine admissibility upon travelers’ arrival. The ESTA application collects biographic information and answers to VWP eligibility questions. Travel Request/Authorization Instructions

Controller's office policy co 02 92.

Purpose: This form is used to request authorization of most official University travel, including travel advances, and prepayments when appropriate (see Travel Advance Policy CO 01 92 ). This form is not required for reimbursement of one-day, in-State trips where no excess is requested.

Forms: The Travel Request/Authorization form is available here to the campus community in an Excel file. 

Completed Forms: The completed form should be retained by the appropriate Department Head after approval unless an airline ticket is involved . The Department Head shall obtain the Chancellor's signature on all out-of-country travel. If a prepayment, including airfare, or advance is being requested, the Travel Request form must be forwarded to campus travel office (x. 4218 ) at the same time the reservation is made to properly process payment.

If a travel advance is requested, the campus travel office (x. 4218 ) must receive the Travel Request/Authorization form at least ten (10) work days prior to the date the travel advance check is required. The department must hold a copy of the Travel Request for attachment to the traveler's Reimbursement form.

Student Academic Travel Reimbursement information can be found on the Academic Affairs website at https://www.uncp.edu/about-uncp/administration/departments/academic-affairs/policies-and-procedures .

Instructions: Follow the numbered items for completing the Travel Request/Authorization form.

l. Name of traveler.

2. Department generating the request.

3. 6-Digit Acct #. List the departmental account or accounts to be charged with the travel expense. If travel is to be paid by multiple accounts, the approved Travel Request is held by the supervising department after all account approval signatures have been obtained until which time it is attached to the traveler's Reimbursement form. The approval for reimbursement requires only the supervising department head signature.

     Examples:

1.   Traveler is a Biology professor who is not the Chairman.The professor will be traveling from the funds of the (a) Teaching and Learning Center, (b) Faculty Development Grant, and (c) the Biology Department - all three (3) 6-digit accounts will be listed on the Travel Request and all three (3) approving signatures will be on the Travel Request. The Department Chair (in this case the Biology  Dept. Head) holds the Travel Request after receiving all approval signatures. The Travel Reimbursement form requires only the Biology Department Head's signature and the attached approved Travel Request and any necessary receipts.

2.    Traveler is the Department Chair of the Biology Department. The Department Chair will be traveling out of  Biology funds and Teaching and Learning Center funds. Both 6-digit accounts and both approving signatures will be listed on the Travel Request. The Chair must sign the Travel Request/Authorization form as traveler, get the signature approval for the Teaching and Learning Center funds, and forward the Travel Request/Authorization form to his/her supervisor - the Associate Vice Chancellor for Academic Affairs - whose office will hold the Travel Request for later attachment to the Travel Reimbursement form and forwarding to the campus travel office (x. 4218 ) for reimbursement.

5. Date the request is typed.

6. Destination of trip. If guest of University (prospective employee, etc.) is coming to campus, indicate where traveler is coming from.

7. Date and anticipated time of departure.

8. Specific reason for trip.

9. Date and anticipated time of return.

10. Check appropriate box(s) to indicate mode of travel. If \"Other\" is checked, specify travel mode.

11. If traveling by car or van, indicate the name of the driver.

12. If traveling by car or van, provide names of all passengers.

13. This section is included for calculating trip costs. All known or anticipated allowable travel costs associated with this trip should be included in this column, including costs of registration fees, airplane tickets, rental cars, taxis, private cars, airport parking, or other trip costs.

      13a. Do not write in this space; it is reserved for travel office (x. 4218 ) use.

14. Indicate the number of days and estimated cost per day for hotel/motel, if applicable. Lodging costs greater than the rates indicated in CO 02 92A constitute excess lodging and must be indicated by checking the appropriate Special Travel Designation in item 20.

15. Indicate the number of days and allowable cost per day for meals, if applicable. (See CO 02 92A .)

16. If by car, indicate anticipated mileage and the rate per mile. (See CO 02 92A .) If by air, indicate if ticket is billed to UNCP. If rental vehicle is required, attach explanation for need of rental to the Travel Request. (See Policy C0 05 94A ) NOTE: State cars are processed via a Vehicle Request  sent directly to the University Motor Pool.

17. Itemize miscellaneous allowable travel costs. For each item, enter a description and amount.

18. If prepayment of registration fee (must be $75 or more before the University will prepay) is requested, type \"prepayment requested\" by Registration Fee and attach completed copy of registration documentation to Travel Request/Authorization form with payee and mailing address highlighted.

19. Total anticipated expense (steps 14 - 18) and enter total cost.

20. Special Travel Designations. Check all boxes applicable to this travel. The Department Head may require additional justification or documentation prior to authorization of travel. Chancellor's signature is needed only if it is out-of-country travel and/or he is the traveler's supervisor.

21. Signature of traveler.

22. Signature of Department Head or Department Heads approving travel if multiple travel accounts are involved.

23. Amount of funds limited by the Department Head or Department Heads for this trip. NOTE: use only if department is limiting funds available for the trip.

24. Chancellor (if Chancellor is traveler's supervisor and/or the travel is our of country).

25. Advance. Indicate if an advance is required. The traveler must attach a written statement giving explanation for the request. See the Travel Advance Policy Statement (CO 01 92) for a list of situations where a travel advance may be issued. NOTE: The advance is limited to 90% of the total out-of-pocket estimated costs.

26. If a travel advance is requested in item 25, the traveler must sign on line 26. By signing here the traveler is agreeing to the conditions preceding the signature.

See what documents a child needs to travel to or from the U.S. alone or with a parent or relative.

Children traveling to the U.S.

All children, including infants, must have their own travel documents such as a passport or document from a Trusted Traveler Program to enter the U.S. If you travel or are going to travel with a child, consider taking the following documents:

  • If the child is traveling with only one of their custodial parents, they must have a letter of consent, preferably in English and notarized, from the other parent or signed by both parents. The letter should say "I acknowledge that my son/daughter is traveling outside the country with [the name of the adult] with my permission."
  • If one parent has sole custody of the child, a copy of the custody document can take the place of the other parent's letter.
  • Parents who frequently cross the border by land with a minor must always carry a letter of permission from the other parent.

U.S. citizen children traveling abroad

Ports of entry in many countries have security measures to prevent international child abduction . If you are traveling alone with your child, you may be required to present documentation proving you are the parent or legal guardian. You may also need a letter of permission from the other parent for your child to travel. 

If your child travels alone, depending on the country, they may be required to present a notarized letter from both parents or their legal guardian. If a minor is traveling abroad and is not accompanied by both parents or a legal guardian, contact the embassy or consulate of the country you will be visiting and ask about entry and exit requirements for that country.

  • You may ask for a larger passport book with more visa pages, at no extra cost, by checking the 'large book' box at the top of the DS-11.

2. Get Evidence of U.S. Citizenship (and a photocopy)

Your evidence must be an original or replacement copy. The document must have the official seal or stamp of the office which issued it. You must submit one of the following documents for your child:

  • Issued by the city, county, or state of birth
  • Lists applicant's full name, date of birth, and place of birth
  • Lists the parent(s)' full names
  • Has the date filed with registrar's office (must be within one year of birth)
  • Has the registrar's signature
  • Has the seal or stamp of the city, county, or state which issued it
  • Consular Report of Birth Abroad or Certification of Birth
  • Certificate of Citizenship
  • Please note you must also provide a document, such as a birth certificate, that lists the parent(s) or legal guardian(s) of the child. Full validity means the document is or was valid for 10 years for adults and 5 years for children under 16.

Paper only : You cannot submit digital evidence of U.S. citizenship such as a mobile or electronic birth certificate. You must submit physical evidence of U.S. citizenship and a photocopy of the document. 

Returning your child's document : We will return your child's document in a separate mailing up to 8 weeks after you receive the new passport.

Tips for making a photocopy :

  • Black and white (no color)
  • Use 8.5 inch by 11 inch paper
  • Use a single side of the paper

If you do not submit a photocopy, you must submit a second copy of your citizenship evidence. We will keep this copy for our records.

3. Show Your Relationship to Your Child

You must submit a document that lists the parent(s) or legal guardian(s) of the child. Examples include:

  • U.S. birth certificate (also evidence of U.S. citizenship)
  • Foreign birth certificate
  • Adoption decree
  • Divorce or custody decree
  • A court order

Some documents, like a U.S. birth certificate, show  both  U.S. citizenship and parental relationship. These documents must be originals or certified copies (not photocopies).

You and your child may have different last names, as long as the document showing your relationship to your child lists your full name.

If your name is different than the one on the document showing your relationship to your child, submit proof of your legal name change.

4. Get a Photo ID (and a photocopy)

Both parents or guardians must bring a physical, photo ID and a photocopy of it. If your photo ID is from a different state than the state in which you are applying, bring a second photo ID. 

You must show at least  one  of these photo IDs:

  • Valid or expired, undamaged U.S. passport book or passport card 
  • In-state, fully valid driver's license or enhanced driver's license with photo
  • Certificate of Naturalization 
  • Certificate of Citizenship 
  • Government employee ID (city, county, state, or federal)
  • U.S. military or military dependent ID
  • Current (valid) foreign passport
  • Matricula Consular (Mexican Consular ID) used by a parent of a U.S. citizen child applicant
  • U.S. Permanent Resident Card (Green Card) used by a parent of a U.S. citizen child applicant
  • Trusted Traveler IDs (including valid Global Entry, FAST, SENTRI, and NEXUS cards)
  • Enhanced Tribal Cards and Native American tribal photo IDs
  • In-state, fully-valid learner's permit with photo
  • In-state, fully-valid non-driver ID with photo
  • Temporary driver's license with photo

5. Show More Documents (if both parents or guardians cannot apply)

  • Both parents or guardians must approve that we can issue a passport to a child, and go with the child to apply in person.
  • If one or both parents or guardians cannot apply in person with their child, you will need to show more documents.

Important : Submit  Form DS-3053  and other notarized statements within three months of signing them.

6. Provide a Photo

You must provide one photo with your child's application. Go to our  Passport Photo page  for photo requirements and to see examples of photos. 

  • Do not attach or staple your child's photo to the form. The acceptance agent or passport employee will review the photo and staple it to your form.
  • Some  passport acceptance facilities
  • A company which offers photo services
  • Home. Ask your friend or family member to take your child's photo. Print it on glossy or matte photo quality paper. 

7. Calculate Fees

When applying using Form DS-11, you will pay two separate fees - an application fee and an execution (acceptance) fee. You will pay the application fee to the U.S. Department of State, and the execution (acceptance) fee to the facility which takes your application. 

  • Add $60 to your application fee if you want  expedited service .
  • Add $19.53 to your application fee if you want us to ship your completed passport in 1-2 days after we issue it.

*How to fill out your check and pay the application fee to the U.S. Department of State. Please note you must pay a separate execution (acceptance) fee. 

Families may write one check or money order to the U.S. Department of State if they are applying at the same time. The check or money order must include the name and date of birth of each applicant.

8. Find Location to Apply

In the United States:

  • Traveling in more than 3 weeks?  Go to a  passport acceptance facility  such as a post office, library, or local government office. Check with the facility to see if you need to make an appointment. 
  • Traveling in less than 3 weeks?   Make an appointment  to apply at a passport agency or center.

In another country:

  • Contact your  U.S. embassy or consulate .

9. Track Your Application Status

You can  subscribe to email updates  about your application status, and  learn more about each status update .

It may take 2 weeks from the day you apply until your child's application status is “In Process.” 

Frequently Asked Questions

How will you send my child's passport and supporting documents.

You will get multiple mailings. The number of mailings depends on what document(s) you asked for.

Passport Book : You may get your new passport and citizenship documents in two mailings. You may wait 8 weeks after getting your passport before you get a second mailing with your citizenship documents. We will return the passport book using a trackable delivery service.

Passport Card : You may get your new passport card and your citizenship documents in two mailings. You may wait 8 weeks after getting your passport before you get a second mailing with your citizenship documents. We only send the passport card via First Class Mail. We do not send cards using 1-2 day delivery services.

Both a Passport Book and Card : You may get three separate mailings:

  • New passport book
  • New passport card
  • Citizenship documents

Contacting Us : If you have been waiting more than 8 weeks for your documents, call us at  1-877-487-2778  to report that you have not received your documents. 

If you want us to reimburse you for a lost supporting document, you must contact us within 90 days of the date which we mailed your passport. You will also need to provide a receipt to show the cost of replacing the document. 

Can I pay for faster delivery and return shipping?

Yes. You may choose one or both of the following shipment options:

  • Delivering application to us : Pay for Priority Mail Express for faster shipping. The price for this service varies depending on the area of the country.
  • Returning the passport to you : Pay $19.53 for 1-2 day delivery. This means you will receive your passport   1-2 days after we send it. Include this fee with your check or money order payable to the U.S. Department of State. Do not submit a return envelope to us with postage pre-paid. 

You may receive your passport and supporting documents in separate mailings. If you are renewing a passport card, we will send it to you via First Class Mail. We do not use 1-2 day delivery services if you only applied for a passport card.

What countries require Form DS-3053 "Statement of Consent" to be notarized at an embassy or consulate?

In certain countries, a DS-3053 must be notarized at a  U.S. embassy or consulate  and cannot be notarized by a local notary public. Currently, these countries include:

To file a claim, you  must   complete ,  sign , and  submit a Claimant's Statement and Authorization form along with supporting documents for every incident. For answers to the most common claims-related questions, see the FAQs section below and select the topic "Claims."

You can complete and submit the Claimant's Statement and Authorization form and supporting documents in one of the following ways:

You can complete, sign, and submit the Claimant’s Statement and Authorization form online via  Member Portal , our self-service portal for policyholders.

To access Member Portal, you will first need to register an account. Click here to register .

If you have already registered your account, follow the instructions below.

If you have an existing Client Zone or Student Zone account, you do not need to register for Member Portal. Simply sign into Member Portal using your Client Zone or Student Zone log in information.

Haven’t Yet Added Your Policy to Your Member Portal Account?

You will need to add your policy to your Member Portal account before you will be able to submit a claim via Member Portal.

1. Visit Member Portal . Enter the email address associated with your account and then click the “Continue Sign In” button.

3. You will be presented with your Member Portal home page. Click “Claims and Appeals” in the main navigation menu at the top of the page.

Don’t Have Access to the Claims and Appeals Page?

If you selected that you are not the insured or the insured’s legal guardian upon registration, you will not have immediate access to the Claims and Appeals page. Instead, you will see the Protected Health Information Authorization page. To gain access to this section of Member Portal, enter the insured’s email address and click the “Send Request” button.

The insured will receive an email with a link to complete, sign, and submit the HIPAA form via DocuSign. You will gain access to the Claims and Appeals section and will be able to submit a claim for the policyholder within 3-5 days of the date the policyholder completed and submitted the form.

To file a new claim via DocuSign, click the "Submit New Claim Via DocuSign" button under the "Claimant's Statement and Authorization Form" subheader in the "File a Claim" section.

5. Enter your name and email address and click the “Begin Signing” button.

6. Click the white box next to “I agree to use electronic records and signatures.” Then click the yellow “Continue” button to the right.

7. Fill in all the required information (as indicated by the red text boxes) and sign everywhere a signature is required (as indicated by the yellow “Sign” boxes).

Attach your supporting documents ( medical records, receipts, itemized bills, etc.), if necessary, in the appropriate section via the yellow attachment boxes. If you have more than 3 attachments or exceed the 5MB attachment limit.

8. When you are done, click the yellow “Finish” button in the top righthand corner.   Clicking “Finish” submits your documents and sends a PDF of your signed Claimant’s Statement and Authorization form to your email.

  • Download or print a copy of your completed form for your records. If you do not receive a confirmation email with your completed form attached within 24 hours, please contact WorldTrips to make sure your form was properly submitted.

Need to Attach More Than 3 Supporting Documents OR Submit Additional Supporting Documents After You Have Already Submitted Your Claim Via DocuSign?

Return to the Claims and Appeals page of the Member Portal and click the “Submit Additional Supporting Documents” button under the “Submit Additional Supporting Documents (If Needed)” subheader in the “File a Claim” section.

You will land on the Submit Additional Supporting Documents page. Use the dropdown to choose your claim number. (If you have just submitted your Claimant’s Statement and Authorization form and do not yet have a claim number to select, you can still submit your documents without selecting a claim number.) Then upload your files and click the “Submit” button.

Once your documents have been successfully submitted, you will receive a confirmation email with your policy number, confirmation of the types of documents submitted (for a claim or appeal), your claim number (if selected), and the number of documents you submitted.

  • Download the  Claimant’s Statement and Authorization form here .

Important Note If You Are Filing This Claim on Behalf of Another Person

If you are filing this claim on behalf of another person, you MUST fill out the section of the Claimant's Statement and Authorization form labeled "Supplement D - Authorization Form for Use and/or Disclosure of Protected Health Information." (This section of the claim form is also referred to as a HIPAA form). This section must be signed by the policyholder in order for us to speak with you regarding any of the policyholder's Protected Health Information (PHI). Without this signed form, we may only disclose to you the status of the claim.

Already Submitted Your Claim and Did Not Complete Supplement D?

You do not need to submit a whole new claim form.  See your options for submitting the Authorization Form for Use and/or Disclosure of Protected Health Information (HIPAA form) separately here .

Important Note for Students

For eligibility purposes, you must submit the following along with a Claimant’s Statement and Authorization form:

  • A copy of your education-related visa (F-1, J-1, OPT, etc.) or valid I-20/DS2019
  • Proof of full-time student status (not necessary if submitting a valid F-1 visa including OPT, or J-1 visa)

Important Note Regarding Upfront Payments

If you paid for medical treatment upfront and wish to be reimbursed for an approved claim electronically, make sure you complete the appropriate section of the Claimant's Statement and Authorization form titled "Supplement C -  Payment Authorization Agreement Form." If you do not make your selection and fill out the information for your preferred payment type, you will receive a paper check for reimbursement of any eligible expenses to the name and address listed on file.

If you have already submitted your claim and did not fill out the "Authorization Agreement Form - Wire Payments” section, you do not need to submit a whole new claim form. You may instead fill out and submit the Payment Authorization Agreement Form with your payment preference via DocuSign within  Member Portal .

Sign in and click “Claims and Appeals” in the main navigation. Then click the “Fill Out and Submit Via DocuSign” button under the “Payment Forms” subheader in the “File a Claim” section.

Enter your name and email address and click the “Begin Signing” button. Click the box next to “I agree to use electronic records and signatures.” Then click the yellow “Continue” button, select your preferred payment type, fill in the required information (as noted by the red boxes), sign the form, and click the yellow “Finish” button.

How Do I Register for a Member Portal Account?

1. Visit Member Portal at  https://worldtrips.my.site.com/MemberPortal . 2. Under “Continue Sign In” click “First-time user? Register for an account.”

3. On the Register page, enter the email address you wish to use to sign into your account. This email does not need to match the email address associated with your policy. Enter your policy number and select whether or not you are the insured or the insured’s legal guardian. Enter the insured’s first and last name as entered on the policy. (You can see how the insured’s name was entered on your ID card and fulfillment documents that were sent to you upon purchase.) Enter the insured’s date of birth. Then choose a password. Follow the password instructions noted on the page.

Read and agree to the terms and conditions by checking the box. Then click the “Continue” button.

  4. You will be taken to the Sign In page. Enter the email address and password you used to register your account and click “Sign In.”

5. To add an additional layer of security when signing into your account, you will be prompted to choose your preferred multifactor authentication method.

Options Include: a.  Okta Verify  - Click "Setup" if you wish to use a push notification sent to the mobile app

When and How Will I Receive My Fulfillment Documents?

During the checkout process, you will have the option to receive your ID cards and other fulfillment documents by:

If you select “Email only,” you will receive your ID cards and fulfillment documents by email as soon as your application has been processed and your credit card has been approved. This typically takes a few minutes or less.

There may be situations when you choose to appeal how a claim was processed. You may appeal your claim decision using one of the following methods:

What Are Atlas Group International and Atlas Group America?

Atlas Group policyholders who are U.S. citizens or non-U.S. citizens (and home country is not the United States) traveling outside of the U.S. will be covered under Atlas Group International, which provides coverage outside of the United States.

Atlas Group policyholders who are non-U.S. citizens traveling to the United States will be covered under Atlas Group America. (For the purpose of this American visitor insurance coverage, the United States also includes Puerto Rico and the U.S. Virgin Islands.)

Are Any Pre-Existing Conditions Covered by a StudentSecure Insurance Plan?

Charges resulting directly or indirectly from any *pre-existing condition are excluded from StudentSecure Elite and StudentSecure Select during the first 6 months of coverage and from StudentSecure Budget during the first 12 months of coverage. All StudentSecure plans include **Acute Onset of Pre-existing Conditions (excluding chronic and congenital conditions) up to a lifetime maximum of $25,000 for eligible expenses. Note that StudentSecure Smart provides Acute Onset of Pre-existing Conditions coverage only.

*Pre-existing Condition means any injury, illness, sickness, disease, or other physical, medical, mental, or nervous disorder, condition, or ailment that, with reasonable medical certainty, existed at the time of application or at any time during the 12 months prior to the effective date of this insurance, whether or not previously manifested, symptomatic or known, diagnosed, treated, or disclosed to us prior to the effective date, and including any and all subsequent, chronic or recurring complications or consequences related thereto or resulting or arising therefrom.

**An acute onset of a pre-existing condition is a sudden and unexpected outbreak or recurrence that is of short duration, is rapidly progressive, and requires urgent care. The Acute Onset of Pre-existing Condition benefit will only apply if all of the following conditions are met: 1. A pre-existing condition means an illness, disease, or other condition during the lookback period immediately prior to your effective date of coverage for which you: 1. received a test, examination, or medical treatment; 2. received a recommendation for a test, examination, or medical treatment; or 3. took or received a prescription for drugs or medicine.

Item (3) above does not apply to a condition which is treated or controlled solely through the taking of prescription medication and remains treated or controlled without any adjustment or *change in the required prescription throughout the lookback period before your coverage effective date.

*Change in required prescription means the dosage or frequency of a medication has been increased, reduced, stopped, and/or new medications have been prescribed due to the worsening of an underlying condition that’s being treated with the medication, unless the change is: 1. between a brand name and a generic medication with comparable dosage; or 2. an adjustment to insulin or anti-coagulant dosage. Your Atlas trip protection plan will not cover pre-existing conditions that existed during the lookback period, unless you qualify for a waiver of the pre-existing conditions exclusion. What Supporting Documents Should I Include With My Claim?

For a Trip Cancellation Claim : -

  • Itinerary/invoice for all trip arrangements
  • Proof of payment for trip arrangements
  • Proof of cancellation of all trip arrangements
  • Documentation supporting your reason for cancellation (e.g., completed attending physician’s statement, death certificate and obituary, jury summons, etc.)

For a Trip Interruption/Travel Delay Claim:

  • Documentation of the date you joined the trip and/or left the trip
  • Receipts and proof of payment for any additional expenses claimed
  • Documentation supporting your reason for interruption/delay (e.g., medical notes/records, death certificate, evidence of delay or cancellation by your common carrier including the reason for the delay or cancellation, etc.)

For an Emergency Accident & Sickness Medical Expense Claim:

  • Itemized invoices
  • Medical notes/records
  • Proof of payment for medical expenses incurred during the trip
  • Explanation of Benefits (EOB) from your primary health insurer (if coverage is excess)

For a Baggage Damage or Loss Claim:

  • A copy of your claim with the carrier that had possession of the items when they were lost/stolen
  • A copy of the resolution of the claim with the carrier
  • Itemized list of all items claimed, including the approximate age/date of purchase for each item claimed and proof of the replacement cost for a similar item from a retailer
  • If loss was the result of theft, a copy of the police report filed at the time of loss

For a Baggage Delay Claim:

  • Property Irregularity Report (PIR) issued by the common carrier (shows evidence of baggage delay)
  • Documentation of the date your baggage was returned to you

Many providers are willing to bill us directly, and we are happy to work directly with the provider. The provider should submit original itemized bills to WorldTrips. You, the patient, will still need to submit a completed Claimant's Statement and Authorization form to WorldTrips. If the provider requires you to pay for medical treatment at the time of service, you will need to file a claim for reimbursement. When seeking treatment, please ensure that you provide your name in the same format as provided for your insurance policy. You can check your name format on your fulfillment documents, ID card, or in Member Portal .

At the time of service, ask your medical provider for an itemized bill with the following information: 

  • Provider name and address
  • Provider tax ID (if U.S.-based)
  • Your name, date of birth, and certificate ID number
  • Itemized charges
  • DX (diagnosis) code and CPT (Current Procedural Terminology) code
  • Request your receipt for payment and complete medical records from the visit.

Don't Have Access to the Claims and Appeals Page? If you noted that you are not the insured or the insured’s legal guardian, you will see the Protected Health Information Authorization page in place of the Claims and Appeals page. If You See the Protected Health Information Authorization Page: - Because you selected that you are not the insured or the insured's legal guardian upon registering for Member Portal, you do not yet have access to the Claims and Appeals page. - Under the "Protected Health Information Authorization" header, enter the insured's email address and click the "Send Request" button. - The insured will receive an email with a link to complete, sign, and submit the HIPPA form via DocuSign. You will gain access to the Claims and Appeals page and will be able to see the status of any pending claims within 3-5 days of the date the policyholder completed and submitted the form.
If You See the Claims and Appeals Page: - Click the "Fill Out and Submit Via DocuSign" button under the "Authorization Form for Use and/or Disclosure of Protected Health Information (HIPPA)" subheader in the "File a Claim" section. - Enter the insured's name and an email address and click the "Begin Signing" button. - Click the yellow "Continue" button, fill out the form and the required information (as noted by the red boxes), have the policyholder sign the form, and click the yellow "Finish" button. - Clicking "Finish" submits the document and sends a PDF of your signed Authorization Form for Use and/or Disclosure of Protected Health Information to the email address entered. In health insurance, subrogation is the process through which an insurance company seeks to recover medical costs from another party’s insurance. For WorldTrips, it refers to those times when you or your family may have submitted a claim for medical care for which a third party is responsible. These types of claims could be car accidents, slip and fall claims, negligence claims, or product liability claims. In cases where a claim for services in the United States is in question, WorldTrips is responsible for administering the claim.

We use Equian, an independent company, to assist us with the investigation and recovery process for these subrogated claims. [equian logo?] - What Equian Will Do? As part of the investigation process, you may receive a letter or email directly from WorldTrips and Equian asking you to provide details regarding an incident or paid medical claim. The date of your consultation or treatment and the name of the healthcare provider will be listed on the letter and the investigation will be allocated an event number.
