Our Poconos residential treatment facility is open 24/7/365. This location serves female clients in the beautiful Poconos Mountains region of Pennsylvania, providing confidential care no matter your mental health concerns. We accept most commercial insurance providers, as well as a majority of self-pay financing options.
Clients will experience a multi-modal, evidence based approach that consists of process group, individual and family therapy, psychoeducation, pharmacotherapy, nutritional support and holistic and experiential modalities. The clinical treatment is based in acceptance and commitment therapy (ACT) and incorporates trauma-informed care, motivational interviewing, emotion focused family therapy (EFFT) and dialectical behavior therapy (DBT).
Programming at Tapestry Poconos also includes experiential therapy, a holistic approach which uses expressive activities to help individuals re-experience and process emotions from the past. We offer a range of experiential therapies, including life skills training, nutrition and culinary programming, music therapy, art therapy, horticulture therapy, salt cave excursions and equine-assisted therapy.

The staffing rating takes into account differences in the levels of residents' care needs in each nursing home. For example, a nursing home with residents that have more health problems would be expected to have more nursing staff than a nursing home where the residents need less health care.
Health Inspection
Medicare assigned rating based on weighted score from recent health inspections. More stars mean lower health risks.
Certified nursing homes must meet standards set by the federal government to protect residents. Examples of these standards include:
- Preventing abuse, neglect, and exploitation
- Hiring enough quality staff to provide adequate care
- Managing medications properly
- Protecting residents from physical and mental abuse
- Storing and preparing food properly
State survey agencies conduct health inspections about once a year on behalf of the federal government and may inspect nursing homes more often if the nursing home is performing poorly, if there are complaints, or if the facility reported incidents. The health inspection team consists of trained inspectors, including at least one registered nurse.
Using the federal government’s standards, the inspection team looks at many aspects of life in the nursing home including, but not limited to:
- The care of residents and the processes used to give that care
- How the staff and residents interact
- The nursing home environment

Lexington Court is a nice quiet nursing home and rehabilitative facility tucked away in Richmond's far west end off of Gayton Road. You can also reach it via Ridgefield Parkway. The staff are friendly and help. If a resident gets sick and has to be hospitalized then you don't lose your bed while you're out. If you come for rehabilitative services and decide to stay for long term care then you will have first choice over someone that isn't there which is awesome. This facilities medical and physical therapy staff are not contracted but in house staff that works with this facility. Therefore, you get to know the staff. There isn't different contracted staff coming in out daily where you don't get the chance to know. If ever you're looking for a facility, be sure to call the admissions director, Doyle Hull for assistance and a tour.

Horrible! Patients begging for help & wait btwn 20 minutes- 2 hrs before anyone responds to call bell. Place is filthy & unsanitary. I can't believe this place is allowed to operate. As you walk through the newly renovated hallways into the patient's room hallway you will be hit with the smell of urine, feces and a mix of some perfume smell to cover up- it's disgusting! The rooms claim to be renovated but are dark, depressing and filthy. Beds are old and noisy with NO safety features! Bedpans are not lined & rinsed out after use, then put on filthy floor or in drawer where patients clothes. Reused again on patients bed. There are two patients to a room ( many patients are permanent), and one toilet is shared by two rooms ( that is 4 patients sharing 1 toilet!!!). Several days went by and toilet/sink (outside) was never cleaned. Poop like stains on hallway floor remain there for a week or longer. Spills in hallways stay for hours or until they dry up. Staff can be extremely loud/disruptive to patients trying to rest. Nurses in general are friendly, but over stretched (meds can be found on floor) so patients may not get everything needed. If you go down patient hallways you will notice many unattended in wheelchairs get stuck & blocking hallways while staff tries to get around. One person fell out of wheelchair and sat on the floor for over 1/2 hour!!! Unsanitary conditions & practices (use 1 dirty rag to wipe down entire space, just wiping if you get someone to clean patient room). If they are passing out food trays (which are all the same food for all patients unless you want a salad or sandwich, you don't get a choice). If you ring for help they will eventually come in and tell you absolutely no patient care when food trays are being handed out. Terrible! The place is not run well and lacks efficiencies & the needed care for patients. Again cannot recommend or believe this place is allowed to operate. Dept of health needs to send someone there as a patient for a while to see what really goes on. Looks can be very deceiving! When you 1st walk in, newly renovated, but ask for a tour & insist to go to the hallway with rehab inpatients.
The nurses and staff are wonderful, compassionate people but there is not enough of them! You could use the nurse call button and not get a response for 45 minutes but usually over an hour. Soil yourself? That's a good 90 minutes of sitting in it until they help you. The food is horrible. The smell is there. The cleanliness is bad. This place had been shut down before, the opened with a new name. Don't go there or send anyone you care about there.
Unattentive, under staffed. 2-weeks and still no bed rails, no bedside potty. Food is poor quality and nearly nonexistent (see photo). Patient has not improved, is discouraged and wants to go home.
August 15, 2023 Dinner
Yelp moderators said my prior review wasn't specific enough so here you go: Our families experience was not positiveat Centerbury Rehab. My 50 year old brother with dementia who had been there for almost a year on the lockdown unit was starting to show signs of aggressive behavior. There were two incidents early in this stage and I asked for my brothers medication's to be adjusted. No communication from Canterbury staff except to report aggressive behavior. No plan to adjust anything. Fast forward a few months and I get a call from MCV emergency room that my brother I've been dropped off and left by Canterbury for a psych eval, due to aggressive behavior and striking others. It was determined that my brother was not psychotic and he was sent back to Canterbury , two days later, Canterbury sent him back to the emergency room for another psych eval, due to aggressive behavior and being physical and try to climb out of a window which we told them when we first saw the place they weren't secure. At this point the hospital determined there was no change in his baseline dementia, and that he was to return to Canterbury . Canterbury refused to accept my brother back. $80,000 in medical expenses paid by the taxpayers of Virginia. I thought it was a joke but two months later, I receive a 30 day notice of eviction. I got an ombudsman and we contacted the commonwealth. In that legal proceeding they were found in the wrong. Canterbury had every right to evict an aggressive and now violent patient. But there are rules on how to do that properly and they ignored them all. They also ignored my calls, emails and faxes. The care STAFF here is awesome. Lead Admin and the directors need to be replaced by people with empathy and knows the rules of a 190 bed facility.
My 87 year old mother fell and received 30 stitches and staples on the top of her head at St Mary's Emergency Room. The doctor recommended she stay at a rehabilitation facility in order to receive physical therapy for her balance. A bed was available at Canterbury Rehab Center so she was transferred there. Instead of beginning physical 24 hours after admittance she laid in bed. She was not given compression socks to wear and as a result got a blood clot in her lower right leg. There was incompetence on every level of care. She saw one doctor during her 18 days in the center, the call button for the nurses station didn't work, no telephone was in her room, the nurses station telephone would ring 20 times with no one answering important questions regarding her care. The doctor never returned calls. The only saving grace through her stay was the PT department who actually answered questions and was concerned about her recovery. The worst part in all of this "rehabilitation" facility is that her stitches and staples were never removed after we were told they were removed. It was discovered during home care they were still in her scalp! The standard of care at this facility is horrendous! Do not send your loved ones there to suffer. Do the research for their safety and health.
My 90 year old mother recently fell and broke her back. Canterbury Rehab was not our first choice but it was what was available. And it was NOT a good alternative. It definitely did not smell clean, and the nursing care given there (when it was given) was sub par. My mother could not answer questions or advocate for herself, so she was often ignored. She was in the same shirt for six days. It took me pitching a fit to get her a bath. She was not given pain meds regularly and the food did not look particularly appetizing. I definitely do not recommend it. It was a relief to move her to a much better facility today.
That place is horrible, my brother was there and ended up in the hospital twice with UTIs because of his catheter. There were some hard working Nurses and CNAs that did all they could because that place is so understaffed. One part of it has an overpowering smell of urine that never goes away. Hopefully none of your loved ones will end up in that place.
My aunt died at this facility in April 2019 after a stay in the rehab setting. When she was moved to the long-term care part of the facility, things changed. She was in a Medicaid bed and those patients are definitely treated differently than the rehab patients. When she was moved to her long-term care room, the bed she was put into was broken, the floor was filthy, used medical waste was under the bed, there was a leak in the sink onto the floor in the room. The room was far too small for three patients especially as one of them, my aunt, was in a wheelchair. We had to ask to get help with these things and the social worker cleaned up some of this herself. I had to check to make my aunt actually got her medication. On one occasion, was told by the nurse that she had gotten her pain medication just a few minutes ago and could not have any more for several hours. Because she clearly was in pain, I pushed the issue and another staff checked the medication record which showed no pain medication since 9.00 in the morning This was 4.00 in the afternoon. Had I not questioned this further, my aunt would have been left in pain for several hours longer if not more. There were two other patients in this tiny room. They were all Medicaid patients. I then found out that Lexington Court provides TVs for everyone in the hospital except Medicaid patients. To be on Medicaid you must have almost zero financial assets. So, the people least able to afford a TV set are left without them. I bought one for my aunt. One patient in the room, Crystal, was bedridden. She could not do anything except stare at the ceiling and to some extent use her hands. She was quite a bit younger than the other patients. I asked the nurse about her situation and was told she had no visitors and no family. I said I would buy her a TV set if they promised to keep it for her use even if she were moved. That night, my aunt died. The next day it was the hospice company that called to tell us she had died. No one from Lexington court called with information about her death or even with condolences. When I went to her room hours later, she was still in her bed with curtains drawn. Her roommate cried as she told me she heard my aunt struggling for breath in the night and called for help several times. She said she finally had to get out of bed and use her walker to get to the nurse's station before anyone would respond. I took my aunt's TV set and set it up for Crystal, who cried with appreciation. I also gave her some of my aunt's clothing and supplies as there was not one article of clothing in Crystals' bureau. While some of the staff there, the physician, his assistant and some of the nurses and aids were wonderful, they could not make up for the lack of management and order of the place. The cheapness of the management in providing TVs for all except the poorest patients stands out in my mind when I think of this facility. As my aunt and I experienced this was a truly dismal way for her life to end.
My mother is currently here after a hip surgery. Stay away!!! My mother has been here for about 3 weeks. It's impossible to get to a human on the phone. They will bounce your calls around and then finally send your call to a voicemail and it will be full so you can't leave any messages. The entire 3 weeks she's been here it's one thing after another. They just recently moved her into another wing on Friday evening, and it's been 24 hours without a tv or anything to listen to or watch. I was told by the nurses station it would be 3 days to get a tv in her room because maintenance isn't available on the weekends. So her and her roommate are sitting in silence so far all weekend. They told me to call Saturday morning and talk to a supervisor, yet I was transferred and their voicemail was full. There have been many ongoing issues with meals and medications and lack of attention.There have been other issues as well. I understand Covid has caused many issues, but when I can't get any human on the phone after many attempts it's really frustrating. I would love to be contacted regarding my mothers terrible experience here.

PGA TOUR winner Grayson Murray has passed away at the age of 30. Murray, who recently celebrated his first PGA Tour victory in seven years at the Sony Open in Hawaii, faced significant challenges throughout his career.
Despite his young age, Murray had already achieved notable success in the world of golf. He won his first PGA Tour title at the Sony Open in Hawaii earlier this year, earning himself a spot in the Masters and other prestigious events for 2024, including the Players Championship . His victory also came with a substantial prize of $1.49 million.
Murray's journey to success was not without its obstacles. In 2016, he won on the Korn Ferry Tour in his rookie season, narrowly beating Cameron Smith. However, he also faced personal challenges, including struggles with alcohol. Murray made a significant decision to change his life, stating, "I'm a different man now. I would not be in this position today if I hadn't put down the drink eight months ago."
A turning point in Murray's life came in 2022 when he was involved in a motorcycle crash in Bermuda during the PGA Tour's Bermuda Championship . The accident required 50 stitches, half of which were on his face. Reflecting on the incident, Murray admitted, "I should have died. If I didn't have my helmet on, I probably would have died. It should have been my rock bottom, but I had to keep going."
Following the accident, Murray voluntarily entered rehab for a month, realizing it was time for a change. He credited his recovery to the support of his loved ones and his faith, stating, "The Lord was looking out for me then, and he's definitely looking out for me now."
Despite his challenges, Murray remained optimistic about the future, expressing his hope to inspire others who may be facing similar struggles. He appreciated the positive aspects of his life, including his fiancée, parents, and extended family, stating, "My life is so good right now. I wouldn't trade anything."
Grayson Murray's story, though tragically cut short, serves as a reminder of the power of perseverance and the importance of having a strong support system. He leaves behind a legacy of resilience and determination , inspiring those who knew him both on and off the golf course.

Coming to University of Idaho Students Starting June 5, 2024
The University of Idaho provides strong support to students’ mental health through its endorsement of the partnership between the CMHC and BetterMynd. This collaboration enables the CMHC to increase access to counseling services for U of I students enrolled at the Moscow campus, satellite campuses and online programs across and beyond Idaho.
With BetterMynd, students have access to counseling and workshop services at traditional hours (8 a.m. to 5 p.m.) as well as evenings, weekends and university breaks. With a large network of licensed mental health professionals, BetterMynd provides students with the flexibility to select counselors with specialties, demographics, identities, lived experiences or a language that match their preferences.

Services Available at the CHMC While Using BetterMynd
The CMHC empowers students to determine the best fit of services for their mental health needs by offering them the opportunity to see BetterMynd counselors via Zoom or CMHC mental health providers in person or via Zoom. If a student chooses to access counseling and workshop services through the BetterMynd platform, they can still utilize the following services available at the CMHC:

Do i have to be a student to see a bettermynd counselor.
BetterMynd counseling and crisis services are available for students who maintain their active enrollment status at the University of Idaho during the time that they seek mental health services.
Can I See a BetterMynd Counselor and a CMHC Counselor at the Same Time?
You can attend counseling with one clinician at a time to focus on your counseling needs and goals. If you choose to work with a BetterMynd clinician, you can still have access to other services available at the CMHC, including the following:
During the time that you receive one-on-one counseling with a BetterMynd clinician, you are not eligible for one-on-one individual counseling at the CMHC.
What Are Common Issues Can BetterMynd Counselors Help Me With?
BetterMynd counselors are independently licensed mental health professionals. They can offer support and intervention for common concerns related to stress, anxiety, depression and relationships.
BetterMynd counselors conduct an initial evaluation in your first Zoom appointment and will collaborate with you on developing the best course of treatment.
How Many Times Can I See a BetterMynd Counselor?
- Each student has seven free credits per semester to use toward individual counseling
- Each student has three free credits per semester to use toward workshops
- One credit is used for each appointment that is scheduled.
How Does Session Credit System Work?
- Unkept appointments or cancellation within 24 hours are nonrefundable and result in a credit deduction from your available credit pool.
- CMHC reserves the right to terminate your access to BetterMynd services after two unkept appointments. To explore other mental health options, contact the CMHC to speak with the Clinical Director.

How Happens After I Used Up All My Free Credits?
- Use your own health insurance or self-pay to continue with your BetterMynd counselor
- Seeing a CMHC counselor at no cost
- Exploring counseling options in the community
- Request additional sessions by submitting a request completed by your BetterMynd counselor to the CMHC. Extended appointments may be granted on a case-by-case basis.
Can I Use BetterMynd for Crisis or Same-day Appointments?
BetterMynd is not a crisis service. If you are in need for same-day appointments for urgent needs related to mental health concerns can contact the Counseling and Mental Health Services (CMHC) office at 208-885-6716. The CMHC is operated according to the University of Idaho’s operation schedule. During Fall and Spring semesters, the CMHC is open from 8 a.m. to 5 p.m., Monday – Friday. During Summer, the CMHC is open from 7:30 a.m. to 4:30 p.m., Monday – Friday.
After hours, you can access the Protocall at 208-885-6716 (press #3). Additional urgent/crisis support resources are also available on the CMHM webpage.
If you are in imminent danger to yourself and/or others, call 911 or visit the nearest emergency care clinic.
Can I Change My BetterMynd Counselor?
During your registration, you can browse through counselor’s profiles available on your BetterMynd dashboard. In addition, you can select specialties and counselor diversity factors that are important to you. The BetterMynd dashboard will present you with the top three therapists that best match your preferences.
BetterMynd also offers you the flexibility to change your counselor preferences any time you want.
What Should I Do When I Need Documentation from my BetterMynd Counselor?
Contact your BetterMynd counselor directly. The CMHC does not have access to your clinical records with BetterMynd counselor.
What Should I Do When I Need My BetterMynd Counselor to Communicate Information with the CMHC?
The CMHC does not make any contact with your BetterMynd counselor without your written consent. If you would like the CMHC to receive or exchange confidential information with your BetterMynd counselor, we request that you complete and sign a Release of Information (ROI).
Does the CMHC Have Access to My Clinical Records with BetterMynd Counselor?
The CMHC does not have access to your treatment records with BetterMynd counselor as your mental health services with BetterMynd counselor are protected under HIPPA regulations. To request your treatment records, we recommend that you contact you BetterMynd counselor directly.
