Service animals

Flying with a service animal.

Fully-trained service dogs may fly in the cabin at no charge if they meet the requirements.

A service animal is defined as a dog that’s individually trained to do work or perform tasks for the benefit of a qualified individual with a disability, including a physical, sensory, psychiatric, intellectual, or other mental disability, including but not limited to:

  • Visual impairments
  • Mobility impairments
  • Post-traumatic stress disorder

Our team members are trained to ask certain questions to determine if your animal is a service animal acceptable for travel.

Please note, service animals in training, emotional support animals, and comfort animals may travel as pets, not as service animals. All requirements and applicable fees will apply.

Forms and advanced notice

If you're traveling with a service animal on flights operated by American, we recommend that you electronically submit the U.S. Department of Transportation (DOT) Service Animal Air Transportation Form that verifies your service animal's health, training, and behavior to the Special Assistance Desk at least 48 hours before your flight. We’ll notify you as soon as your document is approved.

You can also present the completed form at the airport on the day of travel. Be sure to arrive early.

If your trip includes a flight on a partner airline you’ll need to contact them and complete all required forms for traveling with a service animal.

Flights to the U.S.

The U.S. Centers for Disease Control and Prevention (CDC) has issued a temporary suspension of dogs, including service dogs, traveling to the U.S. from countries considered high-risk for dog rabies.

Only service dogs with an approved CDC Dog Import Permit, or that meet CDC U.S. vaccination and microchip requirements may fly on American.

Flights over 8 hours

If you have a flight over 8 hours in your itinerary, the Department of Transportation Service Animal Relief Attestation Sanitation Form is also required stating your animal won’t need to relieve itself or can do so in a way that doesn’t create a health or sanitation issue.

Instructions

  • Read and complete the DOT Service Animal Air Transportation Form
  • Complete the DOT Service Animal Relief Attestation Form if applicable
  • Submit at least 48 hours before your flight, or present the completed form at the airport on the day of travel
  • Keep the form with you during your trip

Service Animal ID

Once you’ve submitted your form and it’s been approved, we’ll send you an email with your Service Animal ID (SVAN ID). You can add the SVAN ID when booking future travel with this animal and be approved without resubmitting forms. Keep in mind, your SVAN ID will expire after 1 year from the date of signature or when the animal’s vaccination expires, whichever is first.

You can add an SDP ID instead of an American-issued Service Animal ID (SVAN ID) when booking travel with us and be approved without submitting forms.

Travel requirements

  • Animal must be harnessed, leashed or tethered at all times
  • Animals under 4 months of age may not travel as a service animal
  • Animal must be clean and well-behaved
  • Animal must be able to fit at your feet, under your seat or in your lap (lap animals must be smaller than a 2-year old child)
  • Final approval for travel will not happen until you arrive at the airport and it is determined that the animal will safely fit at your feet.
  • If the animal is in a kennel, it must fit under the seat in front of you with the animal in it.
  • An individual is limited to 2 service animals

Animals may not:

  • Be seated in an exit row
  • Protrude into or block aisles
  • Occupy a seat
  • Eat from tray tables

If your animal is too large or too heavy to be safely accommodated in the cabin, you may need to:

  • Rebook on a flight with more open seats
  • Buy a ticket for the animal
  • Transport the animal as a checked pet.

Animal behavior

Animals must be trained to behave properly in public and they won’t be permitted in the cabin if they display any form of disruptive behavior that can’t be successfully corrected or controlled, including but not limited to:

  • Biting or attempting to bite
  • Jumping on or lunging at people

If this behavior is observed at any point during your journey and isn't corrected or controlled, the animal will be considered a pet. All requirements and applicable fees will apply.

If you don’t complete or meet all the requirements, your animal may be able to fly as a pet. All requirements and applicable fees will apply.

Destinations with restrictions

Trained service dogs can travel in the cabin to / from all Hawaiian islands. All other animals (with the exception of service dogs) must first go through Honolulu for acceptance by Hawaiian quarantine officials.

Your service animals must meet our travel requirements as well as the quarantine restrictions and documentation requirements set by the state of Hawaii. You must provide applicable documentation for both at check-in.

Dogs traveling to LHR need approval from the Heathrow Animal Reception Center (HARC). You should contact them as soon as possible, but at least 7 days before your flight. The HARC charges a fee (£410) for animals that do not meet the criteria for assistance animals.

British Airways doesn’t accept emotional support animals on any of its flights.

To fly to LHR:

  • Notify the HARC via email
  • If approved, a letter will be sent
  • Print the letter and provide it at check-in

To fly with service dogs, email Scotland’s animal travel agency at least 48 hours before your flight. You'll need to carry an ID card or valid certificate from the organization that trained or supplied the animal.

Service dogs need pre-approval letters from the Animal Reception Center in your destination city. You’ll need to travel with the letter and any required health documentation.

Service dogs can travel in the cabin to / from Ireland. All dogs must be fully compliant with the rules for entering Ireland. You’ll need to notify the Irish Authorities in advance to arrange a mandatory veterinary compliance check upon arrival.

Service dogs can travel in the cabin to / from India. All dogs must be fully compliant with the rules for entering India. You’ll need to notify the Indian Authorities to obtain a No Objection Certificate (NOC) at least 7 days ahead of travel.

Service and emotional support animals require an animal import permit from the Guyana Ministry of Agriculture. You must travel with and provide the import permit at check-in.

You’ll need to complete the St Vincent and the Grenadines Animal Import Permit Application and submit it to the St Vincent and the Grenadines Ministry of Agriculture, Forestry and Fisheries. You must travel with and present the import permit and a valid health certificate at check-in for your service dog.

I am traveling with a trained service dog

We welcome fully trained service dogs that are individually trained to perform tasks or work for the benefit of a qualified individual with a disability. Dogs are the only trained service animals we accept. Service animals in training, emotional support animals, and therapy animals may qualify to travel as pets in accordance with our pet policy .

Your trained service dog must be harnessed, leashed, or otherwise tethered to you at all times at the airport and onboard the aircraft. They must also be trained to behave and under your control. A dog that poses a direct threat to the health or safety of others, disrupts cabin service, or engages in disruptive behavior will be denied boarding.

Documentation requirements

You'll need to present a completed digital or printed U.S. Department of Transportation Service Animal Air Transportation form at the ticket counter or gate on your day of travel. The form must be dated on or after the date you purchased your ticket. A service animal vest, harness, ID card, or registration certificate will not be accepted in place of the form.

At the airport

You are required to check in with a Customer Service Agent at the ticket or departure gate counter to present the required form. Our Employees may also ask fact-finding questions to determine whether your dog is a trained service dog or eligible to be accepted as a pet, including asking what work or task your dog has been trained to perform.

Onboard the aircraft

You may sit anywhere onboard the aircraft except for in an emergency exit seat . Your dog must be sitting at your feet or in your lap (if it is no larger than a child under the age of two). If your dog is using a carrier, it must be stowed under the seat in front of you for taxi, takeoff, and landing.

Your dog is not permitted to block any exits or exit paths, extend into the aircraft aisle, occupy an aircraft seat, occupy a tray table, or encroach upon a neighboring seat.

Additional information

Acceptance restrictions, policies, and documentation requirements for trained service dogs vary by destination when traveling to Hawaii , Puerto Rico , or internationally . You're responsible for researching and complying with the animal travel and acceptance laws, requirements, and procedures of each location on your itinerary.

Things to know about traveling with service animals on JetBlue:

  • JetBlue accepts trained service dogs only. 
  • JetBlue requires all service animals to be under the control of the handler at all times—in the airport and on the plane—and harnessed, leashed, or otherwise tethered. 
  • The behavior of all service animals will be assessed at the airport to ensure safety requirements are met.   
  • Notification of service animal travel must be completed at least 48 hours prior to your flight.
  • We recommend starting this process as soon as you book your reservation. 
  • If you have booked your flight within 48 hours of departure you may complete the US DOT Service Animal Air Transportation Form at the airport with a Complaints Resolution Official. Please arrive early and we will provide the accommodation if we can do so by making reasonable efforts, without delaying the flight.
  • Service animals in training are not accepted for travel on JetBlue. 
  • If you are traveling with a large service animal or two service animals, your animal(s) must fit within the footprint of the seat(s) you have purchased. If your animal(s) is too large to fit in the seat(s) foot space you have purchased, you may purchase additional seat(s) to guarantee travel or wait for a flight that has sufficient empty seats available. 
  • No animal is ever allowed to occupy a seat. 
  • All animals must remain on the floor, unless the animal is small enough to fit fully on the traveler's lap without touching any part of the seat, tray table, or nearby travelers. 
  • Upon request, assistance will be provided by a JetBlue crewmember to and from the animal relief area. Please ask an airport crewmember for assistance.  

Notification of service animal travel must be provided up to 48 hours in advance of the first originally scheduled departure time. We recommend providing notification as soon as you book your reservation. You may choose the best process for your travel from the following options.

If your trip includes a flight on a partner airline, you'll need to contact them separately as their process may be different. 

Additional documentation may be required at the airport:   

  • Puerto Rico and the U.S. Virgin Islands require vaccination documentation for the animal to be admitted into the destination.    
  • It’s your responsibility to have the required documentation for your service animal(s).  
  • If one of the two notification processes above has not been followed, the customer may bring or request the US DOT Service Animal Air Transportation Form to the airport for an onsite review/assessment with a Complaints Resolution Official (CRO).

JetBlue permits service animals to accompany you in any seat except the emergency exit row, unless the animal obstructs an aisle or other area that must remain unobstructed in order to facilitate an emergency evacuation.   

  • In our Mint seating, you will forfeit the lie-flat feature in order to accommodate your service animal on the floor.  
  • If your service animal is small enough to fit fully on your lap without touching any part of the seat, the lie-flat feature may be used.   
  • All animals must remain on the floor, unless the animal is small enough to fit fully on the customer's lap without touching any part of the seat, tray table or nearby travelers. 
  • If you’re traveling with a large service animal or two service animals, your animal(s) must fit within the footprint of the seat(s) you have purchased and not prevent a nearby traveler from utilizing the amenities of their seat. If your animal(s) is too large to fit in the seat(s) foot space you have purchased, you may purchase additional seat(s) to guarantee travel or wait for a flight that has sufficient empty seats available. 
  • JetBlue requires all service animals to be under the control of the handler at all times—in the airport and on the plane—and harnessed, leashed or otherwise tethered.

In accordance with the CDC guidelines on dog importation from certain countries, and taking into account JetBlue network limitations, JetBlue will not accept service animals on flights to or from:

  • Colombia (BOG, CTG, MDE)
  • Ecuador (UIO)

 for vaccinated animals.

  • Dominican Republic (SDQ, STI, PUJ, POP)
  • Ecuador (GYE)
  • Guatemala (GUA)
  • Guyana (GEO)
  • Haiti (PAP)
  • Honduras (SAP)

Important note: Customers may travel with service animals to the above destinations from any JetBlue city.

Dogs entering from the above-listed cities must provide the following documentation in order to be cleared for travel to the US:

  • Have a valid US-issued rabies vaccine certificate OR CDC Dog Import permit
  • Have proof of microchip
  • Be at least 6 months old
  • Be healthy upon arrival

For other international destinations, documentation requirements vary by country. Customers are responsible for understanding each country's animal import requirements and acquiring all necessary documentation before traveling with a service animal.

Inform the TSA officer about your service animal.

Screening in Standard and TSA PreCheck® Lanes

You and your service animal will be screened by the WTMD. You may walk through together or you may lead the animal through separately on a leash. If you alarm the WTMD, you and your service animal will undergo additional screening, including a pat-down.

For metal detector screening, please consider using items (harness, vest, or leash) on your service animals that have little to no metal.

If you opt not to be screened by the WTMD, you and your service animal will undergo a pat-down.

TSA will not separate you from your service animal.

Accessories

Service animal collars, harnesses, leashes, backpacks, vests and other items are subject to screening. Please consider using items that have little to no metal. Items that are necessary to maintain control of the service animal do not need to be removed.

If you need to relieve your service animal and exit the security checkpoint, you and the service animal will need to go through the screening process again. Request to move to the front of the line upon your return.

Medication for service animals must go through X-ray or inspection screening. Please separate medications and inform the TSA officer that you carry these items for your service animal.

How to submit a request to travel with your service dog

Your trained service dog.

You can travel in an Air France cabin with your service dog for free, granted you comply with current international regulations. We recommend submitting all requests at least 48 hours* before your flight's departure. If you submit your request after this time, all documentation checks and request processing will be handled at the airport on your departure day. To be allowed on board, your service dog must:

  • Be well-trained.
  • Comply with all the health requirements of your departure, arrival, and connecting countries.
  • Wear a harness at all times.
  • Stay securely fastened to your seat or seat belt.
  • Refrain from obstructing the aisles or the space allocated to other passengers.
  • Have irreproachable behavior in all circumstances.
  • Not occupy a seat or obstruct emergency exits.
  • Be ready to wear a muzzle if necessary for the safety of passengers and crew. 

We strongly suggest that your service dog be dressed to indicate its service dog status (with either a vest, identification tag, or a visible sign on the harness). In addition to the behavioral conditions your service dog must meet, please also note the following administrative and medical travel requirements:

  • If you have a snub-nosed dog, please note that they are permitted in the cabin, but are not allowed in the hold for Air France flights due to medical reasons. Please consult your veterinarian before considering air travel with a snub-nosed animal.
  • If you need to give your service dog medication onboard to ensure their comfort, please remember to bring their medical prescription.
  • Except on flights to or from the U.S., you must bring your dog's service training certification, specifying their service, and a copy of their passport page with breed, weight, and size information.

* 96 hours if you are traveling to or from the United Kingdom or United Arab Emirates.

How to submit the documents To submit the official documents, you can provide hard copies at check-in. You can also email or send them by mail ahead of time. 

  • For flights from the United States:

You submit your request to travel to the UK with your service dog 96 hours before your flight's departure to ensure that a “Pet checker”* can be at the airport upon your arrival. In addition to all the documents required for entry into the UK, only service dogs trained by a recognized organization affiliated with the International Guide Dog Federation (IGDF) are accepted. Assistance dogs traveling to the UK must be trained by an organization that meets all the criteria defined by Assistance Dogs International (ADI). Upon arrival in the UK, you and your service dog are not allowed to disembark the aircraft until the Pet Checker from the airport screening company performs identity and documentary checks on your dog. For more information on the requirements for transporting your dog to this destination, please consult the UK government website . * Airport staff responsible for checking that your pet is legal to enter the UK.

To travel with your trained service animal to the United Arab Emirates, please get in touch with our Saphir assistance service at least 96 hours before your flight's departure to allow for the necessary required document verification by airport and government authorities in your destination country.

Regulations:

Per international regulations, we only accept service dogs trained by a certified and recognized organization on our flight. Non-service dogs or service dogs in training without their trainers are accepted in the cabin in a bag or crate. They are also accepted in the hold, unless they are snub-nosed animals. Per applicable transport law, Air France reserves the right to hold you financially liable for any damage your service dog may cause.

The ADA explains what businesses and state/local governments must do to make sure that they do not discriminate against a member of the public with a disability who uses a service animal.

Generally, businesses and non-profits that are open to the public as well as state/local governments must allow service animals to go most places where the public can go. This is true even if they have a “no pets” policy.

About Service Animals

Service animals are:.

Any breed and any size of dog

Trained to perform a task directly related to a person’s disability

Service animals are not:

Required to be certified or go through a professional training program

Required to wear a vest or other ID that indicates they’re a service dog

Emotional support or comfort dogs, because providing emotional support or comfort is not a task related to a person’s disability

If the dog’s mere presence provides comfort, it is not a service animal under the ADA. But if the dog is trained to perform a task related to a person’s disability, it is a service animal under the ADA. For example, if the dog has been trained to sense that an anxiety attack is about to happen and take a specific action to help avoid the attack or lessen its impact, the dog is a service animal.

Examples of Service Animal Tasks

A person who uses a wheelchair may have a dog that is trained to retrieve objects for them .

A person with depression may have a dog that is trained to perform a task to remind them to take their medication .

A person with PTSD may have a dog that is trained to lick their hand to alert them to an oncoming panic attack .

A person who has epilepsy may have a dog that is trained to detect the onset of a seizure and then help the person remain safe during the seizure .

Where Service Animals Can Go

Generally, service animals are allowed to be with their person, even in places that don’t allow pets. For example, service dogs can go into:

  • Restaurants

EXAMPLE: A restaurant offers indoor and outdoor seating. A woman arrives at the restaurant with her service dog and asks to sit inside. The restaurant cannot require the woman to dine outside because of her service dog.

The ADA also applies to certain types of housing, including:

  • Housing at public and private universities
  • Public housing programs run by state, county, and city governments
  • Emergency shelters

The Fair Housing Act applies to many types of housing, both public and privately owned, including housing covered by the ADA. Under the Fair Housing Act, there may be different rules that apply when a resident or applicant with a disability uses a service animal or other animal to assist with their disability. The U.S. Department of Housing and Urban Development is responsible for administering the Fair Housing Act. Other laws apply to airplanes

Asking if a Dog is a Service Animal

If you are working at a business or state/local government facility and it is unclear to you whether someone’s dog is a service dog, you may ask for certain information using two questions .

You may ask:

Is the dog a service animal required because of a disability?

What work or task has the dog been trained to perform?

You are not allowed to:

Request any documentation that the dog is registered, licensed, or certified as a service animal

Require that the dog demonstrate its task, or inquire about the nature of the person’s disability

Because service animals are not required to wear vests, a dog that is wearing a vest is not necessarily a service animal. The dog still needs to be trained to perform a task for a person with a disability to be a service animal.

When a Service Animal Can Be Kept Out

A business or state/local government does not need to allow a service animal if the dog’s presence would fundamentally alter the nature of the goods, services, programs, or activities provided to the public.

In most settings, a service animal will not fundamentally alter the situation. But in some settings, a service dog could change the nature of the service or program. For example, it may be appropriate to keep a service animal out of an operating room or burn unit where the animal’s presence could compromise a sterile environment. But in general, service animals cannot be restricted from other areas of the hospital where patients or members of the public can go.

Asking Someone to Remove Their Service Animal

A business or state/local government can ask someone to remove their service animal if:

  • The dog is not housebroken.
  • The dog is out of control, and the person cannot get the dog under control.

Can i pay for faster delivery and return shipping.

Yes. You may choose one or both of the following shipment options:

  • Delivering application to us : Pay for Priority Mail Express for faster shipping. The price for this service varies depending on the area of the country.
  • Returning the passport to you : Pay $19.53 for 1-2 day delivery. This means you will receive your passport   1-2 days after we send it. Include this fee with your check or money order payable to the U.S. Department of State. Do not submit a return envelope to us with postage pre-paid. 

You may receive your passport and supporting documents in separate mailings. If you are renewing a passport card, we will send it to you via First Class Mail. We do not use 1-2 day delivery services if you only applied for a passport card.

What are passport expeditors and courier companies?

Passport expeditors and couriers are companies that are not part of the U.S. Department of State. These companies submit applications and pick up passports for customers. 

These companies may charge several hundred dollars in extra fees. They may also charge for services that we offer for free such as making an appointment and filling out a form.

Requesting a Refund for Expedited Service

Service Commitment

If you paid the $60 fee for expedited service when you applied and you did not receive expedited service, you may be eligible to  request a refund  of this fee. We cannot refund any other passport fees or your travel expenses if you miss your trip.

Non-Refundable Fees

We cannot refund the passport application fee and the execution fee. By law, we collect both fees and keep them even if a passport is not issued.

Title Transfers and Changes

To prove vehicle ownership, it’s important to have a valid, up-to-date, and accurate California Certificate of Title. Here’s how you can transfer and change a title.

How to Transfer a Title

Anytime there’s a change to a vehicle or vessel’s registered owner or lienholder, that change must be updated in DMV’s records within 10 days and the California Certificate of Title must be transferred to the new owner.

A change in ownership is usually due to:

  • Sale, gift, or donation
  • Adding or deleting the name of an owner
  • Inheritance
  • Satisfaction of lien (full payment of car loan)

To transfer a title, you will need:

  • Either the California Certificate of Title or an Application for Replacement or Transfer of Title (REG 227) (if the title is missing). 
  • The signature(s) of seller(s) and lienholder (if any).
  • The signature(s) of buyer(s).
  • A transfer fee .

Depending on the type of transfer, you might need to complete and submit additional forms. See below for other title transfers and title transfer forms.

Other Title Transfers

When you’re buying a new car or a used car from a dealership, the dealer will handle the paperwork and you’ll receive your title from DMV in the mail.

When vehicle ownership is transferred between two private parties, it’s up to them to transfer the title. If you have the California Certificate of Title for the vehicle , the seller signs the title to release ownership of the vehicle. The buyer should then bring the signed title to a DMV office to apply for transfer of ownership. 

If you don’t have the California Certificate of Title , you need to use an Application for Replacement or Transfer of Title (REG 227) to transfer ownership. The lienholder’s release, if any, must be notarized. The buyer should then bring the completed form to a DMV office and we will issue a new registration and title.

Make sure you have all signatures on the proper lines to avoid delays.

Other Steps for the Seller When Vehicle Ownership is Transferred

  • 10 years old or older.
  • Commercial with a GVW or CGW of more than 16,000 pounds.
  • New and being transferred prior to its first retail sale by a dealer.
  • Complete a Notice of Transfer and Release of Liability (NRL) within 5 days of releasing ownership and keep a copy for your records.

Once the seller gives the buyer all required documentation and DMV receives the completed NRL, the seller’s part of the transaction is complete.

*If the vehicle has been sold more than once with the same title, a REG 262 is required from each seller.

Other Steps for the Buyer When Vehicle Ownership is Transferred

  • Current registered owner(s), how names are joined (“and/or”), and lienholder/legal owner (if any).
  • License plate number, vehicle identification number (VIN), make, model, year, and registration expiration date.
  • Title brands (if any).
  • Words “Nontransferable/No California Title Issued,” indicating a California title was not issued and a REG 227 cannot be used (see FAQs).
  • Get a smog inspection (if applicable).

Once the buyer has provided the DMV with all the proper documents and fees, the vehicle record is updated to reflect the change of ownership and a registration card is issued.

A new title is issued from DMV headquarters within 60 calendar days.

To transfer a vehicle between family members, submit the following:

  • The California Certificate of Title properly signed or endorsed on line 1 by the registered owner(s) shown on the title. Complete the new owner information on the back of the title and sign it.
  • A Statement of Facts (REG 256) for use tax and smog exemption (if applicable).
  • Odometer disclosure for vehicles less than 10 years old.
  • Transfer fee .

You may transfer a vehicle from an individual to the estate of that individual without signatures on the Certificate of Title.

Submit the following:

  • The California Certificate of Title. On the back of the title, the new owner section must show “Estate of (name of individual)” and their address. Any legal owner/lienholder named on the front of the title must be re-entered on the back of the title.
  • A Statement of Facts (REG 256) confirming the owner is deceased and Letters Testamentary have not been issued. The person completing the statement must indicate their relationship to the deceased.

Use tax and a smog certification are not required.

Vehicle ownership can be transferred to a deceased owner’s heir 40 days after the owner’s death, as long as the value of the deceased’s property in California does not exceed:

  • $150,000 if the deceased died before 1/1/20.
  • $166,250 if the deceased died on or after 1/1/20.

If the heir will be the new owner, submit the following to a DMV office:

  • The California Certificate of Title. The heir must sign the deceased registered owner’s name and countersign on line 1. The heir should complete and sign the back of the title.
  • Affidavit for Transfer without Probate (REG 5) , completed and signed by the heir.
  • An original or certified copy of the death certificate of all deceased owners.

If the heir prefers to sell the vehicle, the buyer also needs (in addition to the items above):

  • Bill of Sale (REG 135) from the heir to the buyer.
  • Transfer fee (two transfer fees are due in this case).

To transfer vessel ownership, submit the following:

  • The California Certificate of Ownership. The registered owner signs line 1. The legal owner/lienholder (if any) signs line 2. Complete the new owner information on the back of the certificate and sign it.
  • Bill(s) of sale, if needed to establish a complete chain of ownership.
  • A Vessel Registration Fee .
  • Use tax based on the tax rate percentage for your county of residence.

After you sell a vessel, complete a Notice of Transfer and Release of Liability (NRL) within five days of releasing ownership and keep a copy for your records.

How to Update or Change a Title

Because a California Certificate of Title is a legal document, it is important to keep it accurate and up-to-date. Here’s how you can update or change a title. 

Order a Replacement California Certificate of Title

You must order a replacement California Certificate of Title when the original is lost, stolen, damaged, illegible, or not received. 

To order a replacement title, submit the following:

  • Application for Replacement or Transfer of Title (REG 227) .
  • The original title (if you have it).
  • California photo driver license (if submitting form in person).
  • Replacement title fee .
  • If another replacement title was issued in the past 90 days, a Verification of Vehicle (REG 31) completed by the California Highway Patrol (CHP). This requirement only applies if the registered owner’s name or address doesn’t match DMV records*.

You can submit your application either in-person* at a DMV office or by mail:

If you’re submitting your form to a DMV office, we recommend you make an appointment so you can avoid any lines. 

You’ll receive your title by mail 15-30 calendar days from the date you submit the replacement title application.

*If you’re applying for a replacement title and the registered owner’s name or address doesn’t match DMV records (except for obvious typographical errors), you must submit your application in person with proof of ownership (e.g. registration card) and an acceptable photo ID (e.g. driver’s license/ID card).

Change or Correct a Name on a Title

Your true full name must appear on your vehicle or vessel California Certificate of Title and registration card. If your name is misspelled, changes (e.g as a result of marriage or divorce), or is legally changed, you need to correct your name on your title.

To change or correct your name, submit:

  • California Certificate of Title with your correct name printed or typed in the “New Registered Owner” section
  • A completed Name Statement in Section F of the Statement of Facts (REG 256) .

Removing Information that was Entered by Mistake

If a name or other information is entered on a title by mistake, complete a Statement to Record Ownership (REG 101) .

Savvino-storozhevsky monastery and museum.

Savvino-Storozhevsky Monastery and Museum

Zvenigorod's most famous sight is the Savvino-Storozhevsky Monastery, which was founded in 1398 by the monk Savva from the Troitse-Sergieva Lavra, at the invitation and with the support of Prince Yury Dmitrievich of Zvenigorod. Savva was later canonised as St Sabbas (Savva) of Storozhev. The monastery late flourished under the reign of Tsar Alexis, who chose the monastery as his family church and often went on pilgrimage there and made lots of donations to it. Most of the monastery’s buildings date from this time. The monastery is heavily fortified with thick walls and six towers, the most impressive of which is the Krasny Tower which also serves as the eastern entrance. The monastery was closed in 1918 and only reopened in 1995. In 1998 Patriarch Alexius II took part in a service to return the relics of St Sabbas to the monastery. Today the monastery has the status of a stauropegic monastery, which is second in status to a lavra. In addition to being a working monastery, it also holds the Zvenigorod Historical, Architectural and Art Museum.

Belfry and Neighbouring Churches

Located near the main entrance is the monastery's belfry which is perhaps the calling card of the monastery due to its uniqueness. It was built in the 1650s and the St Sergius of Radonezh’s Church was opened on the middle tier in the mid-17th century, although it was originally dedicated to the Trinity. The belfry's 35-tonne Great Bladgovestny Bell fell in 1941 and was only restored and returned in 2003. Attached to the belfry is a large refectory and the Transfiguration Church, both of which were built on the orders of Tsar Alexis in the 1650s.

To the left of the belfry is another, smaller, refectory which is attached to the Trinity Gate-Church, which was also constructed in the 1650s on the orders of Tsar Alexis who made it his own family church. The church is elaborately decorated with colourful trims and underneath the archway is a beautiful 19th century fresco.

It was built between 1404 and 1405 during the lifetime of St Sabbas and using the funds of Prince Yury of Zvenigorod. The white-stone cathedral is a standard four-pillar design with a single golden dome. After the death of St Sabbas he was interred in the cathedral and a new altar dedicated to him was added.

Under the reign of Tsar Alexis the cathedral was decorated with frescoes by Stepan Ryazanets, some of which remain today. Tsar Alexis also presented the cathedral with a five-tier iconostasis, the top row of icons have been preserved.

Tsaritsa's Chambers

The Nativity of Virgin Mary Cathedral is located between the Tsaritsa's Chambers of the left and the Palace of Tsar Alexis on the right. The Tsaritsa's Chambers were built in the mid-17th century for the wife of Tsar Alexey - Tsaritsa Maria Ilinichna Miloskavskaya. The design of the building is influenced by the ancient Russian architectural style. Is prettier than the Tsar's chambers opposite, being red in colour with elaborately decorated window frames and entrance.

At present the Tsaritsa's Chambers houses the Zvenigorod Historical, Architectural and Art Museum. Among its displays is an accurate recreation of the interior of a noble lady's chambers including furniture, decorations and a decorated tiled oven, and an exhibition on the history of Zvenigorod and the monastery.

Palace of Tsar Alexis

The Palace of Tsar Alexis was built in the 1650s and is now one of the best surviving examples of non-religious architecture of that era. It was built especially for Tsar Alexis who often visited the monastery on religious pilgrimages. Its most striking feature is its pretty row of nine chimney spouts which resemble towers.
