In 2020, there was a noticeable decrease in the number of health tourists due to the global pandemic. 435,691 patients preferred Turkey to receive healthcare services. Tourism revenues from foreign visitors and non-resident citizens coming for health tourism and medical reasons decreased to USD 1,371,189 thousand in 2020.

In 2021, a total of 729,592 people received health services and the revenue generated from these services amounted to USD 2,016,262 thousand.

In 2022, a total of 1,381,807 people came to Turkey to receive health services and the revenue generated from these services amounted to USD 2,206,750 thousand.

In 2023, a total of 1,538,643 people came to Turkey to receive healthcare services and the revenue generated from these visits amounted to USD 3,006,092 thousand.

In the first quarter of 2024, 428,072 patients received healthcare services and the revenue generated from these services amounted to USD 849,663 thousand.

The clinical branches most preferred by international patients were gynecology, internal medicine, ophthalmology, medical biochemistry, general surgery, dentistry, orthopedics and traumatology.

‘Turkey has achieved great success in health field over past 2 decades’

Turkey is one of the most preferred countries in health tourism with its highly qualified medical staff, fast and effective health care services, world-class technological and medical equipment, affordable price advantage, thermal underground resources as well as traditional Turkish hospitality, according to an expert.

"Turkey has achieved great success in the field of health in the last 20 years,” Prof. Murat Dizbay, deputy chief physician and international health office manager at Gazi University Hospital in the capital Ankara, told Anadolu Agency on the occasion of Public Health Week celebrated every year between Sept. 3-9 in Turkey.

“Turkey, which has been closely following the recent technological innovations around the globe, has become a leader in its region with its specialized health professionals,” Dizbay said.

Patients from many countries across the world as well as neighboring countries visit Turkey to receive treatment, he noted, adding Turkish institutions, both in the private sector and in the public sector (university and state hospitals), have achieved significant successes in the field of medicine.

Today, there is annual traffic of 30 million people who go out of their countries as part of health tourism in order to receive quality health services with more affordable prices and shorter waiting times. Health tourism expenditures in the world are expressed with nearly $500 billion, International Health Services Inc. (USHAS), an institution of the Turkish Health Ministry, said on its official website.

Turkey’s target for 2023 in health tourism is to reach 1.5 million health tourists and $10 billion in health tourism income, according to USHAS.

Fast and effective health care services

In recent years, Turkey has risen to the top among the most preferred countries in the world with its rapidly developing health tourism potential.

Underlining that Turkey has a deep-rooted history in providing health services, Dizbay said spa and thermal facilities in the country have been attracting people from all around the world for thousands of years.

“Good results are achieved when the high-quality health service delivery is combined with friendliness and hospitality,” Dizbay noted.

The fact that international patients are offered shorter waiting times for diagnosis and treatment while receiving health care services is also an important factor that makes Turkey a leading country in the health sector, he stressed.

The rapid transfer of the latest technology used in the world to the country and affordable price advantage compared to developed countries make Turkey a center of attraction for health tourism, he said.

Dizbay added that the natural and historical beauty of the country also offers patients who visit the country for treatment the opportunity to have a holiday.

Impact of COVID-19 on health tourism in Turkey

The coronavirus pandemic has caused serious problems in the health sector in Turkey as well as all over the world, Dizbay said.

He noted that there was a significant decrease in the number of international patients after flights from countries whose citizens frequently visit Turkey was canceled during the full lockdown periods.

The increase in the burden of Turkish hospitals and the overwork of health workers during the pandemic period have also negatively affected health tourism in the country, he said.

Dizbay, however, underlined that Turkey has managed the recent crisis successfully thanks to its strong health system.

This success has also attracted the attention of patients and health care providers in other countries and led to a rapid increase in the number of patients visiting the country, according to Dizbay.

He said he expects the increasing trend in the number of patients coming from abroad will continue in the future.

The interest and investment of Turkey’s national government in health tourism is very high, he said, adding that in recent years, an increase has been observed also in the promotion of activities related to health tourism at the national and institutional level.

Health Tourism Data

In 2002, the share of health expenditures in tourism revenues of Turkey was 1%. However, this rate increased to 4.5% in 2020, according to data from USHAS.

In 2019, 662,087 patients received health services as part of health tourism. The amount of tourism income from foreign visitors and citizens residing abroad visiting for health tourism and medical reasons was more than $1.06 billion.

In 2020, 388,150 patients preferred Turkey to receive health services as the global coronavirus pandemic caused a remarkable decrease in the number of health tourists. The amount of tourism income decreased to around $549 million.

In the first quarter of 2021, 110,716 people received health services, and the amount of income obtained was more than $196.73 million.

The most preferred clinical branches by international patients are respectively gynecology, internal diseases, ophthalmology, medical biochemistry, general surgery, dentistry, orthopedics and traumatology, infectious diseases, and otorhinolaryngology.

USHAS was established in 2019 with its headquarters in Ankara to promote the services offered in the field of international health services in Turkey to support and coordinate the activities of public and private sectors towards health tourism.

Turkey’s medical tourism industry boosts visitor numbers

When Turkey’s traditional tourism industry dried up, the country turned to its medical sector to attract a new wave of visitors

Turkey is typically celebrated by global travellers for its white sand beaches, azure seas and opulent palaces. Now, the Eurasian country is seeking to bolster its reputation in the field of medicine, too. Turkey has seen an influx of medical tourists over the past 10 years, rising from 75,000 visitors in 2007 to more than 700,000 in 2017, according to the Istanbul International Health Tourism Association . It is estimated that this number had topped one million by the end of 2018.

The low cost of treatment remains the most attractive factor for the majority of visitors, particularly those from countries with no paid-for state medical system

Faced with soaring healthcare costs at home, tourists from Europe, the US, Russia and Saudi Arabia are opting to fly thousands of miles to save up to 90 percent on the cost of treatments – and incorporate a summer holiday into their trip.

Healthcare in Turkey has come on in leaps and bounds in recent years, and the country now boasts a number of internationally accredited, state-of-the-art hospitals. However, there are some institutions that compromise on safety standards to scrimp on costs – those are the ones that thrifty tourists must watch out for.

Foreign bodies Medical tourism is one of Turkey’s fastest-growing industries and is estimated to contribute $4bn to the country’s economy annually. Patients flock in from across the globe for a range of medical procedures, from the cosmetic to the essential, costing anywhere from a few hundred dollars to almost $20,000.

The low cost of treatment remains the most attractive factor for the majority of visitors, particularly those from countries with no paid-for state medical system. In the US, where healthcare costs are eye-wateringly expensive even for those with medical insurance, it’s a no-brainer to go abroad for treatment. For example, the average fee for a heart valve replacement in the US is $150,000, whereas it’s just $17,000 in Turkey.

Shorter waiting times are a contributing factor, too. Patients can wait up to 18 months for a knee replacement, particularly in the UK, where the overstretched NHS is suffering from a lengthy backlog. By contrast, in Turkey, most patients can have the operation in less than two weeks from the point of booking. It’s a simple equation for many: the less time they spend waiting for a procedure, the sooner they can be back on their feet.

Boosting the numbers Seeking to capitalise on the influx of medical tourists, Turkey has recently unveiled a roster of state incentives to make healthcare even cheaper for overseas visitors. In March 2018, the government passed a law offering a VAT exemption to non-resident foreigners who undergo medical treatment or rehabilitation in an institution accredited by the Ministry of Health. It has also made healthcare the central focus of its national tourism development plan and is aiming to bring in two million patients and $20bn in revenue annually by 2023.

It’s not just patients that will benefit from this prioritisation, either. In April 2018, former Turkish finance minister Naci Ağbal announced that the government was in the process of debating new regulations to encourage private investment in the healthcare sector. He told state-run news agency Anadolu: “We will offer additional incentives to private companies for their health tourism investments.”

Ağbal also suggested that the country was pressing to sign contracts with foreign social security institutions to bring more elderly patients to Turkey for medical care.

This allows Turkey to maintain a fairly consistent flow of tourist trade, rather than the summertime boom that the country has historically seen. It’s a more consistent income model, with fewer external factors – such as poor weather – to dissuade visitors from travelling.

“To try and spread demand more evenly, Turkey and other destinations with similar seasonality issues try and diversify their product,” said Rochelle Turner, Research Director at the World Travel and Tourism Council. “By focusing on new and niche markets where the experience is more important than the weather, these destinations are hoping to attract new visitors, open up regions and create new products that will appeal.”

Overall, tourism has fallen dramatically in Turkey over the past three years due to a series of diplomatic incidents and an increased terror threat dissuading prospective visitors. The country now purports to have smoothed over the political cracks, claiming to be back on a more even keel. The influx of medical tourists indicates a restoration of international confidence and is set to help Turkey replenish the funds it lost as a result of reduced tourism revenue.

Turner said: “750,000 is a significant number of people – all who will be staying in hotels, buying food and drink, and perhaps experiencing some of Turkey’s vibrant culture. This all brings needed income to the country, creating jobs and opportunities for further development through the supply chains of these businesses.”

Safety first For the most part, undergoing a medical procedure in Turkey is a safe experience, particularly with regards to serious curative procedures in leading hospitals. The Anadolu Medical Centre is Turkey’s most respected institution, boasting an affiliation with the US’ eminent John Hopkins Hospital through its international programme. The hospital is particularly famed for its cancer treatment centre, which is recognised by the Organisation of European Cancer Institutes as providing an extremely high level of care for patients.

In the cosmetic sphere, however, standards are not so high. A February 2018 investigation by UK newspaper the Mirror found that Turkey is the worst location for botched plastic surgery operations on British people, with Marmaris-based surgeon Dr Mehmet Kaya singled out by the paper for causing physical damage to a number of patients.

Kaya was found to be offering cosmetic procedures for an 80 percent discount on the price typically paid in the UK. Over a dozen women have since come forward claiming Kaya has left them with permanent scars as a result of improperly stitched wounds, though a clinic representative told the Mirror “it is not the doctor’s fault”.

Michael Saul is a partner at Cosmetic Surgery Solicitors , the first law practice in the UK to help patients claim compensation for complications resulting from substandard plastic surgery.

He believes that it is highly dangerous to go abroad for any kind of cosmetic procedure, due to inadequate pre and post-operative care and the expectation of financial commitment before a patient has seen the facilities or met their operating physician. “There’s a range in surgical standards from country to country, the qualification process will be different for doctors, so really it’s a lottery,” he told Business Destinations .

From a legal standing, it can be near impossible to claim compensation for substandard surgery administered abroad. “The starting point is you normally have to sue in the country where you have had the surgery,” Saul said. “If that’s in Turkey or other parts of Eastern Europe, there will be language barriers, a no-win-no-fee regime is unlikely to be available, and also the level of compensation awards in those types of jurisdictions are likely to be low, too.”

In an effort to tackle sub-standard clinics, in July 2018, the Turkish parliament approved a bill to establish International Health Services, an organisation that will help regulate and promote the health sector. There are concerns, however, that the promotion element of the organisation’s directive will take precedent, with noble regulatory goals falling by the wayside in favour of greater revenues.

Turkey must now strike the right balance between bolstering its tourism revenue while ensuring that stringent clinical standards are met. If safety is not prioritised, the country risks seeing a dramatic decline in medical tourists over the next few years, as prospective visitors realise that the low cost of surgery simply isn’t worth the risk.

Medical tourism, regarded as a sub-category of health tourism, signifies activities that combine health care with economical, short vacations. The rapid expansion and growth of the medical industry due to globalization, the problems related with health systems in some countries (for example, long waiting periods, increasing costs and dissatisfaction with the quality of service), the increasing awareness of consumers of treatment alternatives, and the dynamics affecting the healthcare industry such as conditions in the European Union (EU), should all be considered in the concept and development of both vacation and treatment elements. ‍

Health tourism, which is divided into three branches, namely, healthcare tourism, tourists’ health and medical tourism, is an industry that is accelerating in importance. It has been providing employment and contributing on a serious scale to the economies of the countries in which it is being practiced.

While the first two types of health tourism have long been practiced in many countries such as Switzerland, Germany, Austria, Hungary, the United States of America, the United Kingdom and in some Caribbean countries, medical tourism only started to be organized in the early 1980s. ‍

To date, the majority of medical tourists have been visiting countries that offer health services of first-rate quality but at lower costs. In these regions, many options such as tour programs are offered in the manner of sightseeing tours, shopping, discovery tours and even sunbathing tours and excursions.

The industry has grown rapidly and the establishments and the countries that provide these health services have already started to arrange for specialized services catering specifically to medical tourists. ‍

In this study, we will delineate the opportunities of medical tourism in Turkey, along with its weak and strong aspects. We will elucidate how medical tourism can contribute to strengthening the country’s economy as well as providing high levels of employment, as we determine goals and objectives for the industry on the international level. ‍

Dimensions of the Worldwide Medical Tourism Industry

Medical tourism has expanded its scope to reach large-scale proportions in the 21st century. Over the past years, individuals from advanced and developed countries such as the USA have been traveling to India, Thailand, Mexico, South Africa, Singapore, Hungary, Turkey and Costa Rica to seek medical attention. Complicated surgeries and dental work, kidney dialysis, cosmetic surgery and organ transplants topped the list of the most sought-after procedures.

It was estimated that in 2002, six hundred thousand medical tourists traveled to Thailand to seek treatment at medical centers in Bangkok and Phuket, while approximately one hundred and fifty thousand foreign patients visited India during that time. Today, India is leading the countries that have been enjoying the greatest portion of benefits resulting from the world’s interest in health tourism.

An increase of 30% has been observed annually in the growth rate of the health industry in India, and the income generated from health tourism is expected to reach two billion euros by the year 2012. The main reason for this popularity is that they had foreseen the potential of this tourism long before the others countries; as well, they had been able to operate with low costs and to make efficient presentations and advertisements about their capabilities.

Thailand and Singapore should also be considered  among those that have received greater shares of the health tourism market. Thailand received one hundred and thirty thousand patients in its Bumrunghad Hospital in the year 2003, and was able to pay for 25% of its health budget through the revenue received from these patients. Singapore had set an objective to welcome one million medical tourists by the year 2007. ‍

Eastern European countries that have been trying to reconcile rapidly with the facts of the capitalist world have also started to benefit from health tourism, by utilizing their European Union (EU) memberships, coupled with their low labor costs. Hungary has obtained an input of two million euros, which enables it to be among the shining stars of the health tourism industry. On the other hand, Belgium has found a significant place in the industry by offering aesthetic surgery and obesity treatments. ‍

The major reason why medical tourism is so attractive to many people is that it offers treatments at a comparatively low cost. Medical tourists can now obtain essentially any type of medical or surgical procedure abroad in a safe and effective manner for a fraction of the cost that they would face in their home countries.

Large price disparities exist across the board for numerous medical and surgical procedures. The cost savings are enormous. European countries, like the rest of the Western World, are faced with some major health problems: an ageing population, the decreasing working hours of health personnel, the increasing workforce population in comparison with the retired population, chronic disorders, the rapidly increasing health costs, and long waiting periods that delay and complicate the treatment process. ‍

Various European countries have altered their health policies due to existing economic problems, excluding health services such as dental treatments, aesthetic operations, eye surgery with excimer laser, and green light prostate surgery from insurance coverage. For these reasons, Europeans have started to seek their treatments in countries where the health service fees are relatively lower. In particular, Central European countries benefit from the services provided by Eastern European countries.

However, Turkey has become an alternative among those countries, especially with the currently decreasing costs of airfare. Some European countries welcome rich tourists to their countries and provide treatments at higher charges while sending their own insured patients to safe countries where the health service costs are lower. In reality, however, their waiting periods are much longer. ‍

There is a great potential for health tourism in Turkey, with around 20 million tourists visiting Turkey each year, 4.5 million Turkish people living abroad, and thousands of foreigners residing in Turkey. Furthermore, Turkish people living abroad have communication problems with foreign doctors and health personnel. Failure or difficulties in expressing themselves and their health problems to the doctors abroad are the most important reasons why they prefer to return to Turkey for the health services.

Another reason why they prefer Turkey’s health services is that the quality and scope of health services in Turkey are equal to those found abroad. First- and second-generation Turkish people living abroad keep close ties with their motherland, and they are not able to establish integration with the foreign society they are living in. These factors exert a great impact on their increasing feelings of homesickness, along with the increase in their ages and the diseases that afflict them.

For these reasons, they frequently return to visit Turkey and stay on for months. It has become obligatory for European insurance companies to cooperate with the hospitals in Turkey especially for cases of HT, diabetes, chronic kidney failure and for follow-up procedures for other chronic diseases.

In other words, it is inevitable that the hospitals in Turkey will provide not only emergency health services to these people, but also other routine and elective health services. Otherwise, insurance companies may think that they would be encountering greater health expenses. ‍

Aside from this, our consanguine living in the Balkans, the Middle Eastern countries and in the lands covered by the former Ottoman Empire also wish to benefit from Turkish medicine as long as their financial status will permit them. After the attacks of 9/11, people from the Middle Eastern and African countries with high financial abilities are no longer able to travel to the Western countries where they used to visit, due to stringent visa and security regulations.

They are actively searching for alternative places where they can receive treatment. For instance, the total amount spent by African patients for treatment abroad is around 17 billion US dollars. Nigerians alone spent one billion US dollars for treatments abroad. ‍

Turkey receives patients mainly from the United Kingdom, the Netherlands, Belgium, Azerbaijan, Russia, Bulgaria, Romania, Kosovo and Syria. Turkey is preferred mostly for eye, dental, prostate and fertility operations. When we evaluate the state of our medical tourism in terms of contributions from Turkish citizens living in Europe, the variety of the services provided have been very high, as shown by annual economic earnings that have yielded more than 100 million euros thus far. ‍

Strengths and Weaknesses of Turkey’s Health Industry

Turkey holds a promising position for a bright future in medical tourism. Many improvements have been recorded in the health aspects of the country, which has been advancing on the road to full membership in the European Union. For one, the number of hospitals that provide health services in accordance with EU standards has increased rapidly.

For about 10 years now, the technological infrastructure, as well as the comfort and quality of services of the hospitals in Istanbul, Ankara and Izmir, have been on the same level as their European counterparts. Moreover, the majority of doctors and other health personnel who are experienced, skilled and knowledgeable reside in these cities. ‍

Aside from the three major cities, there are also hospitals that provide health services at par with European standards in the provinces of Antalya, Konya, Kayseri, Bursa, Adana, Kocaeli, Gaziantep and Samsun. Patients who visited Turkey and benefited from the health services provided in these hospitals, as well as the insurance companies of these patients, have mentioned the advanced technological infrastructure and the vocational experience and knowledge of the doctors in these hospitals as being as high as the ones in Europe.

A major strength of the medical health industry in Turkey is that it has sufficient capacity for domestic and foreign patients. There are many highly qualified doctors in Turkish hospitals, which are equipped with the latest technology and staffed by personnel who provide good and qualified health care.

Private hospitals are top-notch, while almost all hospitals have the ISO 2001 quality management certification. More than ten first-class hospitals have been accredited by the JCI while others are still in the process of accreditation. There are no waiting periods for treatment. ‍

The elements of Turkey’s competitive advantage in health tourism can thus be identified as follows: the high quality of infrastructure and hardware in the hospitals; the high experience levels of the doctors and health service providers at par with European standards; the price advantages in comparison with other countries; the country’s natural and historical prosperity and its unique geographical location as a bridge between Europe and the Middle East; and the appropriate climatic conditions combined with quality tourism administration and well-known Turkish hospitality.  

These factors, coupled with the desire of the almost five million Turkish individuals living abroad to receive their health treatments in Turkey, can be mentioned as Turkey’s strengths in the health tourism industry. ‍

On the other hand, Turkey has some weak aspects as well. Despite its strengths and advantages, there have been insufficient efforts to present and introduce the industry to the international market due to the lack of organization among the industry members. There is also difficulty accessing the right person assigned to medical tourism in the hospitals.

Furthermore, the inadequacy of the translations in the websites of some of these hospitals, and the deficient English language levels of the individuals who have contacts with patients and broker institutions lend an amateurish image to these well-respected institutions and obscure the credibility of their high level of medical standards.

The lack of professional brokers has also been preventing Turkey from obtaining its well-deserved share of the international medical tourism market. In particular, the negative image and perception of Turkey in some countries such as the United Kingdom, Belgium and France is also a matter that has to be taken into consideration.

Other weak points of the industry can be listed as follows: the lack of knowledge of the hospitals with respect to the documentation to be issued to foreign patients, problems with many foreign insurance and assistance companies, and the lack of professional companies that can serve as a link between the health institutions and the medical tourists. ‍

The rapid expansion and growth of the medical industry due to globalization, the problems relative to the health systems in other countries (for example, long waiting periods, increasing costs and dissatisfaction with the services), the increasing awareness of consumers, and the dynamics affecting healthcare such as conditions in the European Union (EU) are some of the factors that should be considered in the concept and development of a medical tourism program consisting of both vacation and treatment elements.

Turkey has a unique position in that it harbors more than enough of these two elements. However, in order to evaluate its medical tourism potential, Turkey should emphasize an organized introduction and presentation of the entire industry, highlighting the topnotch, internationally accredited modern hospitals and the experienced, young and dynamic physicians and health personnel employed in these hospitals, along with the advanced technological facilities and the advantage of affordable prices.

In this introduction and advertisement for which the State must be required to provide aid and support, priority should be given to the countries from which medical tourists mainly originate, such as Europe, the Middle East, Central Asia and Africa. Probably, the first thing to be done is to encourage professional organizations that can link consumers with the medical tourism industry. Turkey has the greatest potential to obtain and attract medical tourists, especially Turkish people with EU citizenships living in Europe.

The realization of this potential depends on a few conditions and terms. The State is required to facilitate conveniences and provide reinforcement (investment incentives for the private sector, presentation and visa conveniences, etc.).

Insurance companies should be convinced to give priority to the first and second generations of Turkish people living abroad who spend most of their yearly vacations in Turkey. These people’s chronic diseases and other health problems should be attended to in a more comprehensive, professional and cost-effective manner. ‍

Renée-Marie Stephano is president and founder of the Medical Tourism Association® and editor-in-chief of Medical Tourism Magazine® and the Health and Wellness Destination Guide Series of books. Ms. Stephano has authored several books from “Developing International Patient Centers, Best Practices in Facilitation,” to “Medical Tourism for Insurers and Employers,” and the most recent, “Engaging Wellness ~ Corporate Wellness Programs that Work.” Her articles have been published in publications all over the world. Most recently she co-authored an article which was peer reviewed and published in Tourism Management Academic Journal. Ms. Stephano is an attorney and specializes in working with governments and hospitals to develop sustainable medical tourism/international patient programs and strategies. To that end she provides consultancy to organizations all over the world. She deals directly with ministers of health, tourism and economic development to establish public-private partnerships that support medical tourism and, at the same time, provide a benefit and return to the local community. Ms. Stephano has performed feasibility studies for cities, nations and hospitals worldwide in which she has relayed opportunities for international expansion, clinical development and affiliations and partnerships. She also consults governments in the development of sustainable medical tourism zones & free healthcare zones. Ms. Stephano is a keynote speaker at international conferences, has spoken at hundreds of events and is a featured source of reference on issues related to health, travel and tourism for media outlets around the world. Dr. Marc Fetscherin is an Associate Professor of Marketing at Rollins College (FL), United States. His expertise is in international marketing with a specialization on marketing strategy, marketing research and branding with a specific focus on global brand management, corporate branding, human brands, consumer brand relationships, and destination branding. Previously, he was a Fellow at Harvard University as well as a researcher at the University of California Berkeley. He taught or teaches MBA and executive MBA courses at various institutions worldwide such as East China University of Science and Technology (China), the Copenhagen Business School (Denmark), Jacobs University(Germany), the University of Lausanne, HEC (Switzerland) or the University of Fribourg (Switzerland). He won multiple teaching and research awards. He has extensive international experience and lived and traveled to over 30 destinations. He speaks 5 languages (German, English, French. Spanish, Russian). Previous his academic career he was a consultant for McKinsey & Company and CEO of a Swiss luxury company.
