Before You Go
Planning a trip outside the United States? Our Travel Advisory Service offers health advice tailored to your destinations, activities, and medical history. The appointments are phone consultations with our travel nurses who are certified by the International Society of Travel Medicine and work under the direction of Kaiser Permanente's Infectious Disease physicians. We recommend your consultation be scheduled at least 4 to 6 weeks before departure to plan for recommended vaccinations and medications. However, even if you are leaving soon, the nurse can help you get needed travel vaccinations and medications.
While You're Traveling
Need care while traveling? Call Member Services. They can help you find the closest in-network care option, whether you're traveling within the United States or internationally.

Learn about our travel questionnaire.
If you’re traveling abroad and need a phone appointment with a travel nurse, complete a Travel Questionnaire in advance. This will save time during your appointment. Each traveler needs to complete a questionnaire.
The Travel Questionnaire is located in your "medical record" on kp.org under "Questionnaires."
The information gathered from the Travel Questionnaire:
- Helps our travel nurses make recommendations about vaccines or other preventive care you may need.
- Becomes part of your medical record.
To complete the Travel Questionnaire, you’ll need the following information:
- Travel itinerary, including layovers
- List of prescription medications you take
- List of vaccines from non-Kaiser Permanente providers
- Your name as it appears on the passport
- Medical history of conditions that may lower your immune system
- Vaccines and/or medications you’ve received from non-Kaiser Permanente providers
- Current weight for all children less than 18 years of age
Each family member who's traveling should complete a Travel Questionnaire. If you've set up caregiver access on kp.org to act on behalf of another family member, you’ll be able to complete the Travel Questionnaire for them.

An emergency medical condition is any of the following: (1) a medical condition that manifests itself by acute symptoms of sufficient severity (including severe pain) such that you could reasonably expect the absence of immediate medical attention to result in serious jeopardy to your health or body functions or organs; (2) active labor when there isn't enough time for safe transfer to a Plan hospital (or designated hospital) before delivery, or if transfer poses a threat to your (or your unborn child's) health and safety, or (3) a mental disorder that manifests itself by acute symptoms of sufficient severity such that either you are an immediate danger to yourself or others, or you are not immediately able to provide for, or use, food, shelter, or clothing, due to the mental disorder.
This information is not intended to diagnose health problems or to take the place of specific medical advice or care you receive from your physician or other health care professional. If you have persistent health problems, or if you have additional questions, please consult with your doctor. If you have questions or need more information about your medication, please speak to your pharmacist. Kaiser Permanente does not endorse the medications or products mentioned. Adult & Family Medicine is a primary care medicine specialty and consists of physicians and nurse practitioners who serve as your primary care provider (PCP), and offer personal, comprehensive and quality care for you and your entire family.
As family medicine physicians, we are trained to care for newborns, children, adults, and seniors, which mean we can provide continuous care for you and your family throughout your lives. Our physicians are board certified in Family Medicine or Internal Medicine – and many are also certified in Pediatrics.
Our physicians work together as a team with our staff of Nurses, LVN’s, Medical Assistants and Patient Services Representatives (PSR’s). In addition, they collaborate with the specialist physicians in other departments and may refer you if needed.

Kaiser Permanente 24/7 Advanced Care Centers
Get quality, coordinated care and under-one-roof convenience 24 hours a day, 7 days a week. When you visit a Kaiser Permanente 24/7 Advanced Care Center, you’ll get care from our own, highly-trained physicians and nurses. They’ll have access to your medical record to help personalize your care. You’ll even be supported by advanced technology like MRI and CT scans, plus X-ray, lab and pharmacy services—all under one roof!
More than Urgent Care. Not quite an Emergency Room. 1
At our 24/7 Advanced Care Centers, you can get help with more serious (but non-life-threatening) issues such as:
- deeper cuts
- broken bones
- severe abdominal pain
- dehydration
If your doctor decides you need additional treatment or observation, you’ll have the peace of mind knowing we can monitor your condition in our on-site “Clinical Decision Unit” for nearly a full day — or transfer you to a hospital if needed.
24/7 Advanced Care Center locations:
Gwinnett Comprehensive Medical Center 3650 Steve Reynolds Blvd. Duluth, GA 30096
Southwood Comprehensive Medical Center 2470 Mt. Zion Parkway Jonesboro, GA 30236
TownPark Comprehensive Medical Center 750 Townpark Lane Kennesaw, GA 30144
Save time and money
Why spend hours and pay more at the ER when it’s not an emergency? 1 With Kaiser Permanente Advanced Care Centers, you can enjoy:
- Shorter wait times: You’ll often get care more quickly than at the hospital ER.
- Lower out-of-pocket costs: Your cost for a visit to an urgent care location will usually be lower than for the ER.
- Coordinated care: You’ll get care from our own doctors who have instant access to your electronic medical records.
- Pharmacy Convenience: You’ll be prescribed drugs that are covered by your plan, 2 and can pick them up without leaving the building.
Affiliated Urgent Care Centers
To make getting care easier, you also have access to about 90 affiliated urgent care centers including certain locations from:
- Children’s Healthcare of Atlanta
- Northside Family Medicine & Urgent Care
- Peachtree Immediate Care
