Taxis in Alaska city no longer accepting Medicaid vouchers

BETHEL, Alaska (AP) — Taxi companies in an Alaska city have decided to stop accepting Medicaid vouchers for payment.

Taxis are a main form of transportation for people who travel to Bethel from area communities for healthcare appointments, KYUK-AM reported Thursday.

Mitchell Forbes, the Yukon-Kuskokwim Health Corporation director of communications, confirmed that all of Bethel’s cab companies have decided not to accept Medicaid vouchers.

“Just in Bethel’s case, there was the one vendor who is accepting the transportation vouchers,” Forbes said. “They are no longer accepting that voucher.”

The corporation was informed last week by Kusko Cab owner Naim Shabani that the Medicaid vouchers were too expensive.

Medicaid’s growing list of regulations was the main reason the company decided to stop accepting vouchers that cover the cost of a ride in the southwest Alaska city on the Kuskokwim River.

Kusko Cab was losing money on the vouchers because of the amount of work required to ensure the company followed regulations, Shabani said.

“There’s a roughly 100-page handbook on being a Medicaid transportation provider,” Shabani said.

Kusko Cab is one of three taxi companies in Bethel and Shabani said the other companies are following Kusko’s lead.

A call to another company, Alaska Cab, was not immediately returned.

The corporation is compensating by offering more shuttle services to Medicaid patients instead of vouchers to hotels where they stay, as well as adding hours to its airport shuttle service for all patients.

Medicaid patients will continue to receive their full benefits, Forbes said.

Cabs in Bethel, a destination for medical appointments, are refusing to take Medicaid vouchers

Taxis are a main form of transportation to and from medical appointments for people who travel to Bethel for healthcare, but cab companies in Bethel have decided to stop accepting Medicaid vouchers for payment.

The Yukon-Kuskokwim Health Corporation got a call last week from Kusko Cab owner Naim Shabani, who said that the Medicaid vouchers were too expensive. Shabani said that Medicaid’s growing list of regulations was the main reason they decided to stop accepting those vouchers.

“There’s a roughly 100-page handbook on being a Medicaid transportation provider. There’s a lot of regulations, so there’s not a specific regulation, but just over the years that they had to implement more and more and again,” Shabani said.

Shabani claimed that the cab company was losing money on the cab vouchers because of the amount of work it took to make sure the company followed regulations. Kusko Cab and Alaska Cab are the two main companies in Bethel. Shabani said Alaska Cab would follow his lead. Alaska Cab did not return a call in time for publication.

YKHC is now offering more shuttle services to Medicaid patients instead of vouchers that would cover the cost of a cab. Mitchell Forbes, the director of communications at YKHC, confirmed that all of Bethel’s cab companies have decided not to accept Medicaid vouchers. He said that Medicaid patients will still see their full benefits.

“Just in Bethel’s case, there was the one vendor who is accepting the transportation vouchers; they are no longer accepting that voucher,” Forbes said.

Forbes said that YKHC has added shuttle service to hotels where Medicaid patients stay in addition to adding more hours to its airport shuttle service for all YKHC patients. He said that will be the solution for the future.

Position Summary:

Medicaid Prior Authorization Specialist I under normal supervision, assists in providing transportation/logistical assistance to Alaska Medicaid Recipients traveling to and from the Yukon Kuskokwim Health Corporation (YKHC) service area by determining if the burden of medical necessity has been met by documenting these findings in detail through the appropriate computer systems.

Level 1 employees perform work under close supervision, may only perform simple or routine aspects of the position, and generally require a good deal of instruction and/or training. Senior lead employees perform high level, complex and responsible tasks within this position and generally work with minimum supervision and follow-up from their supervisor.

The following duties are intended to provide a representative summary of the major duties and responsibilities and ARE NOT intended to serve as a comprehensive list of all duties listed and may be required to perform additional, position-specific duties.

REPRESENTATIVE DUTIES

For Medicaid eligible patients and/or escorts, works with clinical providers to determine if medical necessity has been met prior to transportation services. Also, serves as the liaison between the YKHC and the State of Alaska regarding Medicaid Transportation Services. Gathers all information requested by the Alaska State Medicaid Representative by utilizing various computer systems and by communicating to all parties involved so that all prior authorization requests can be handled in a timely manner.

Work collaboratively with patient on eligibility issues. Must be knowledgeable about when to refer patient to the YKHC Medicaid Enrollment department and/or Alaska Division of Public Assistance (DPA) to move authorization along in an expeditious manner.

This position will draw upon experience and general knowledge; will review documentation or contacts the provider or requestor when necessary in order to prove medical necessity for the authorization.

This position will also ensure that there is appropriate documentation to prove the medical necessity of the travel authorization. All submitted authorizations should reflect the correct CPT and diagnosis codes. All authorizations numbers should be reflective of the dates authorized and should correspond to the services authorized.

This position will coordinate with or will assume the function of Prior Authorization, Provider Inquiry and Claims Resolution units to ensure provider/claims resolution.

  • Answer provider inquires in writing and verbally
  • Submit claims, adjustments, voids, etc., electronically and in paper formats
  • Perform and/or evaluate Prior Authorization on-line updates ensuring that notes are accurate and demonstrate the medical necessity for the authorization approval and/or denial.
  • Perform and/or evaluate, and/or resolve process claims.

Maintain a database of information reflecting problems found and actions taken. Analyze findings and advise supervisor of significant system or provider problems/concerns. Work closely with State entities involved with provider claims. Work closely with YKHC office assistants/ health aides and providers, ANTHC/YKHC Medicaid Travel Office’s and the State of Alaska to coordinate the customers lodging and travel needs. May be required to work outside the traditional work schedule. Performs other duties as assigned.

KNOWLEDGE and SKILLS

  • Knowledge of customer service concepts and practice.
  • Knowledge of the Privacy Act of 1974 and HIPAA Privacy Rule Act of 1966.
  • Knowledge of state, federal and public/private insurance, including Medicaid/Medicare.
  • Skill in accuracy with data entry and the ability to make detailed notes
  • Skill in grammar, spelling, sentence structure and effective business letter writing.
  • Skill in establishing and maintaining cooperative working relationships with others.
  • Skill in operating a personal computer utilizing a variety of software applications and be able to retain multiple passwords.

Position Qualifications:

  • High school diploma or equivalent. Computer literacy required.
  • Non-supervisory – Two (2) years of clerical AND customer service experience. An equivalent combination of relevant education and/or training may be substituted for experience.
  • Basic computer/excel skills.
  • 10 touch experience preferred.
  • Must take the State of Alaska HIPAA test.
  • Copiers, print, fax machines, and multi-line telephones. Be able to access multiple computer programs simultaneously.
  • Be able to work in a fast paced office independently. Be able to multi-task between different computer programs, retaining different passwords as needed.

Decades-old law limits access to Wegovy for Medicaid beneficiaries

Medicaid plans aren't required to cover Wegovy for weight loss and obesity, but some do and others are considering adding it for those uses.

Della McCullough has been struggling with her weight since she was 11, when she remembers her mother telling her she was "big boned." Now 53, she says she's tried supplements, diet and exercise — even an all-fruit diet once. None of them worked.

"Still, I'm not doing well," says McCullough, a semiretired school bus driver in Colorado. "I have done nutrition counseling, trauma counseling, meditation, positive affirmation therapy, and still I am almost 300 pounds again and sad and unhappy."

She is interested in new blockbuster drugs that help people with overweight and obesity.

But she and her husband found themselves on Medicaid for the first time last year. Colorado's Medicaid plan won't cover Wegovy for her, an obesity medicine made by Novo Nordisk that she'd like to try.

Wegovy contains the same active ingredient as Novo Nordisk's Ozempic, a diabetes drug, but Wegovy is specifically approved for obesity and weight loss.

An old law complicates Medicaid coverage

Medicaid doesn't have to cover Wegovy and drugs like it because of a decades-old law excluding medicines that treat anorexia, weight gain and weight loss from required coverage.

McCullough is frustrated. "I'm not ashamed of the fact that I am on state assistance, and I can say that what assistance they provide is not adequate, especially if you do have obesity," McCullough says.

"I have done nutrition counseling, trauma counseling, meditation, positive affirmation therapy, and still I am almost 300 pounds again and sad and unhappy," says Della McCullough, a semiretired school bus driver in Colorado.

Robin Feldman, a professor at the University of California Law in San Francisco , says when the law excluding weight-related drugs was passed in the 1990s, everyone thought diet and exercise were the key to losing weight and keeping it off — even though the evidence doesn't support that. "So in that context, being overweight was viewed as lacking willpower and dedication."

While state Medicaid programs don't have to cover weight loss drugs, 16 do right now.

Cost is a factor limiting coverage. Wegovy's list price is more than $1,300 dollars a month . Even if Medicaid programs get a big discount, which they typically do, the total spending could be significant.

Researchers at Brown University found that people on Medicaid are 27% more likely to have obesity compared with people who have commercial insurance.

And people may take these drugs for years.

"States are always having to grapple with, you know, this investment or that investment," says Kate McEvoy, executive director of the National Association of Medicaid Directors . "There are a lot of other preventive health issues, notably around maternal health, issues for children. And so examining the relative merits of those investments is kind of where states are right now."

More Medicaid plans are considering Wegovy

Some state Medicaid programs, like Minnesota's, cover Wegovy because they're required to under state law. Others, like Louisiana's, don't cover Wegovy at all but cover an old weight loss drug, Orlistat , which is considered less effective.

In North Carolina, Kody Kinsley, the state's Secretary of Health and Human Services says he hopes the Medicaid program will cover Wegovy by this summer.

"We have a kind of a standard policy process," he says. "We go through to consider the actuarial impacts, the negotiation of rebates, the value of adding the drugs, all of which we don't really go through for any other drug because they're required to be covered. But for these, because of that federal carve out, we are going through that process."

He says Wegovy would not be the most expensive drug North Carolina's Medicaid program covers and won't bankrupt it.

Advocates say policies that exclude treatments to help people lose weight need to change. "Research shows us that obesity is a complex chronic disease," says Tracy Zvenyach , director of policy strategy and alliances at the Obesity Action Coalition , a national nonprofit organization that is trying to expand access to evidence-based treatments for obesity. "The science is clear that obesity is driven by powerful biology, not by choice," Zvenyach says.

In Colorado, where McCullough lives, Medicaid doesn't cover Wegovy for weight loss.

However, Wegovy was recently approved by the Food and Drug Administration to reduce risk of cardiovascular death, heart attack and stroke for people with cardiovascular disease and who are overweight or obese. That means Medicaid would be required to cover it for some people since it's not just a weight loss drug anymore.

"In Colorado, Wegovy is currently covered by Colorado Medicaid when it's used to reduce the risk of major cardiovascular events in adults with established cardiovascular disease and either obesity or overweight as approved by the FDA," says Marc Williams, a spokesman for the Colorado Department of Health Care Policy and Financing.

But McCullough doesn't have cardiovascular disease and wouldn't qualify.

She says people with obesity are treated differently from people with other health conditions. I ask her if that felt personal.

"You know, I never would have thought that, but now that I'm in that situation, it does feel very personal," she says. "Like, 'She's just fat,' you know? 'It's her problem. She'll figure something out.' Or 'She'll need to change whatever she does.' "

She hopes the policies eventually catch up to the science.

Bethel hospital warns of layoffs after ‘devastating’ revenue loss due to pandemic

A dramatic drop in patient visits has plunged the Yukon-Kuskokwim Health Corp. into a steep financial decline.

The organization’s leadership warns that the large revenue loss could soon lead to furloughs and layoffs.

On April 6, YKHC announced the first case of COVID-19 in the region.

Two days later, on April 8, YKHC employees received an email from YKHC CEO and President Dan Winkelman, warning that some of them could soon lose their jobs.

“The pandemic,” he wrote, “is devastating our financials.”

Few people are seeking health care: Winkelman wrote that patient visits have dropped by 70%. More than half of YKHC’s revenue comes from Medicaid, and the state of Alaska has canceled essential Medicaid travel from most villages. Normal appointments are not happening either, as the hospital has suspended elective procedures, optometry and dental visits.

YKHC began telemedicine, but usage has been “low,” he said. To bring in the few patients who do come to Bethel, YKHC is having to pay for expensive charters due to cuts in air service.

These hard economic times are occurring despite YKHC receiving funding from the CARES Act. Winkelman said that the funds will only cover “several weeks” of operation. Winkelman wrote that if patient numbers do not ”substantially increase over the next couple of weeks, YKHC will be forced to reduce our expenses significantly — which means a furlough and/or a reduction in force.”

YKHC would not talk to KYUK about this financial situation, or provide more details about potential job losses.

In his email to employees, Winkelman thanked the staff for their months of preparing YKHC to respond to the pandemic. He also wrote that if a surge of COVID-19 patients occurs, “we will have much more work to do and it will be an all hands on deck approach.”

The End Date of your trip can not occur before the Start Date.

Traveler reimbursement is based on the location of the work activities and not the accommodations, unless lodging is not available at the work activity, then the agency may authorize the rate where lodging is obtained.

Unless otherwise specified, the per diem locality is defined as "all locations within, or entirely surrounded by, the corporate limits of the key city, including independent entities located within those boundaries."

Per diem localities with county definitions shall include "all locations within, or entirely surrounded by, the corporate limits of the key city as well as the boundaries of the listed counties, including independent entities located within the boundaries of the key city and the listed counties (unless otherwise listed separately)."

When a military installation or Government - related facility(whether or not specifically named) is located partially within more than one city or county boundary, the applicable per diem rate for the entire installation or facility is the higher of the rates which apply to the cities and / or counties, even though part(s) of such activities may be located outside the defined per diem locality.

Domestic fares include all existing Federal, State, and local taxes, as well as airport maintenance fees and other administrative fees. Domestic fares do not include fees such as passenger facility charges, segment fees, and passenger security service fees.

International

International fares do not include taxes and fees, but include fuel surcharge fees.

GSA has adjusted all POV mileage reimbursement rates effective January 1, 2024.

* Airplane nautical miles (NMs) should be converted into statute miles (SMs) or regular miles when submitting a voucher using the formula (1 NM equals 1.15077945 SMs).
